Identifies and addresses psychosocial, cultural, ethnic and spiritual needs of residents and their families. Interacts with residents, families, co-workers, managers and other staff in a kind, knowledgeable and friendly manner. What you'll do The primary purpose of this position is to establish, organize, direct, and implement the Cardiac Rehabilitation Program in accordance with Federal, State, and local guidelines. The Cardiac Rehabilitation Nurse evaluates & treats patients with a wide variety of cardiac disabilities, in an outpatient setting, helping to achieve risk factor reduction in cardiovascular disease using various therapeutic exercises, activities, and educational classes. The Cardiac Rehab Nurse is responsible for documenting all patient care appropriately in the medical record to comply with the policies and procedures of the department and JCAHO requirements. Job Summary The RN - MDS Coordinator will play a crucial role in ensuring accurate and timely completion of Minimum Data Set (MDS) assessments, collaborating with interdisciplinary teams to enhance resident care plans, and maintain compliance with regulatory requirements.
