Contact families regarding significant decline in resident's status or transfer to hospital documenting contact in the EMR. Make rounds and observe individual residents who are experiencing episodes of acute illness, deterioration in health status, recent injury, recovering from surgery, etc., so as to be acquainted with the resident's status, both physically and emotionally and to ascertain that the staff is rendering proper care. At all times abide by policies of the facility and ascertain that employees under his/her supervision do the same. Supervise serving of prescribed diets and fluid intake (it is acceptable to document only deviations from normal) and report persistent unresolved problems to the DON. Ensure that all medications and treatments are charted by the person administering the medication or completing the treatment on his/her assigned shift (EMR system). Prepare incident/accident reports, events and observations using the EMR system. Ensure that a copy of the medical records or an abstract of them are forwarded with the transfer of residents to a hospital or other health care facility. Detect and correct situations that have a high probability of causing accidents or injuries to residents and/or staff. Assure that work/assignment areas are clean and free of hazardous conditions, i.e. spills, excess carts, etc. and that equipment, tools, supplies, etc. are properly stored while working, as well as before leaving such areas for breaks, meal times, and end of the work day. Assure that established infection control and universal precaution practices are maintained when performing nursing procedures. Follow established safety precautions when performing tasks and when using equipment and supplies. Report all equipment malfunctions and breakdowns to the ADON as soon as possible and keep the ADON informed of supply needs and equipment needing replacement. Licensed Practical Nurse (LPN) Duties and Responsibilities: Provides licensed care to assigned residents as ordered by physician and in accordance with facility, federal, state and local standards, guidelines and regulations Provides and receives nursing reports upon reporting in and ending shift duty hours. Ensures assigned work area (i.e. nurse station, medication cart, medication room, etc.), resident care rooms and treatment areas are maintained in a clean and sanitary manner. Directs and supervises the day-to-day certified nursing assistant services for assigned unit to assure that care is being rendered in accordance with current federal, state and local standards, guidelines and regulations. Provides required resident assessments/observations and interventions, reviewing care plans to ensure appropriate care. Takes and records residents blood pressure and vital signs, administers catheter care, administers IV therapy to residents as ordered within license limitations, and provides respiratory care, e.g. oxygen, nebulizer treatments. Receives and transcribes physician’s orders to resident’s charts, Kardex/Care Medication Administration Record (“MAR”), treatment/care plans, as required. Prepares and administers medications and performs treatments as ordered by the physician. Charts nurse’s notes in an informative and descriptive manner that reflects the care provided as well as the resident’s response to the care. Completes the required forms upon admission, transfer, and/or discharge. Ensures the resident’s environment is as free from accidents and hazards. Immediately corrects/addresses potential safety risks as prudent and warranted. Makes written and oral reports/recommendations concerning the activities of shift as required. Ensures that all aspects of resident care plans are implemented and maintained. Make recommendations for care plan changes to the IDT per professional standards. All other duties, as assigned. Licensed Practical Nurse (LPN) Skills and Abilities: Excellent written and verbal communication skills in English. Ability to multi-task and remain calm under stress. Analytical and problem-solving skills. Understands and follows infection control standards, policies and procedures. Must be able to adapt to a flexible work schedule and occasional overnight travel. Demonstrate basic computer knowledge and ability with an aptitude to learn company applications. Licensed Practical Nurse (LPN) Education and Experience: Current CPR Certification required. Possess a current license to practice in the State as a Licensed Practical Nurse. 1 year of experience preferred. Long-Term care experience preferred. Physical Requirements: Walking, reaching, bending, lifting, extended sitting, grasping, fine hand coordination, pushing and pulling, all with or without the aid of mechanical devices is required. Limited potential for exposure to environmental hazards. Understanding and adherence to company safety standards and protocols required.
