SUMMARY: The primary purpose of this job position is to assist in the planning, implementation, and evaluation of recreational, social, intellectual, emotional and spiritual programs, in accordance with the resident’s assessment and care plan, and as may be directed by your supervisors. ESSENTIAL DUTIES AND RESPONSIBILITIES: Assist in planning, developing, organizing, implementing & evaluating the activity programs & quality assurance program in this facility for the Activities Department. Observe resident attendance, mood, behavior, and degree of involvement so that the facility activities & resident progress are evaluated, by noting, reporting and charting resident behavior. Assume the authority, responsibility & accountability of Activity Assistant/Aide. Ensure that all charted activity progress notes are informative and descriptive of the services provided and indicate the resident’s response to the service. Develop & maintain a good working rapport with other departments within the facility to assure that daily activities can be performed without interruption. Create & maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the facility. Assist in preparing activity rooms/areas for scheduled activity programs. Recommend to your supervisor the equipment and supply needs of the activity department. Assist in cleaning up activity areas after completing activity functions. Follow the care plan in providing daily activities for resident. Purchase department supplies, clothing and personal items for the residents. Maintain the confidentiality of all resident care information including protected health information & follow facility guidelines in reporting non-compliance with said guidelines & treat residents with dignity at all times. Oversee residents engaged in scheduled activities to promote the physical, social, and mental well-being of residents by applying experience in activities and knowledge of diagnoses and conditions of residents. Assists in-room residents by visiting with them, writing letters, running errands, and perform assistance with crafts, projects, etc., as necessary. QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Does complete check of skin condition when dressing, undressing, turning/repositioning, etc. Reports any skin abnormalities to the Staff Nurse. Observes customer’s physical condition, attitude, reactions, appetite, etc., and reports any changes and/or unusual findings to the Nurse/RCC so care plan can be updated. Performs personal services for customer’s (i.e., reading, writing letters, etc.) as time allows. Encourages customers to help themselves and promotes self-care in aiding rehabilitation. Assists with care of dying and expired customers. Records resident food/fluid intake. Completes meal monitor form as indicated. Reports changes in resident’s eating habits to the Staff Nurse. Keeps customers water pitchers clean and filled with fresh water on each shift unless contraindicated. Serves between meals and bedtime snacks. Wears transfer belt at all times and utilizes as needed. Keeps the nurses’ call system within easy reach of the resident. Reports all hazardous equipment and conditions to the Staff Nurse immediately. Participates in the development, maintenance, and implementation of the facility’s Quality Assurance program for the Nursing Department. Performs other related duties as assigned.

Utilizes the nursing process to assess, plan, diagnose, implement, and evaluate nursing care, engaging patients and families through the continuum of care. Monitors patient conditions, adjusts care plans, mobilizes resources, and collaborates with the care team to influence care outcomes. Upholds and promotes a culture of safety. Continuously evaluates patient, team, and unit outcomes, taking action as needed. May administer medications, treatments, and therapies safely and according to clinical protocols and procedures. Physical Requirements and Working Conditions Must be able to sit, stand, walk, lift, squat, bend, reach above shoulders, and twist frequently throughout the workday. Must be able to lift and support the weight of 35 pounds in handling patients, medical equipment, and supplies. Must be able to: push/pull with 30 lbs. of force and perform a sliding transfer of 150 lbs. with a second person present. Must have functional speech, vision, hearing, and touch with ability to use fine hand manipulation skills for IV insertion and other procedures/functions. Maneuver foot pedals on carts or machines. Perform physical safety interventions such as patient restraint and verbal de-escalation, if needed. May be exposed to chemical and hazardous waste as well as blood and body fluids and communicable disease. Therefore, protective clothing and equipment must be worn as necessary. Must be able to respond quickly to changes in patient and/or unit conditions. Physical Abilities Testing may be required.
