Our Heart Surgery Program at Cincinnati Children’s, we treat congenital heart defects in patients from before birth through adulthood. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Patient Care 1. Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es). 2. Provides professional nursing care by utilizing all elements of nursing process. 3. Assesses and evaluates patient’s status by: A. Writing and initiating plan of care B. Regularly re-evaluating patient and family/caregiver needs C. Participating in revising the plan of care as necessary 4. Initiates the plan of care and makes necessary revisions as patient status and needs change. 5. Uses health assessment data to determine nursing diagnosis. 6. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process. 7. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care. 8. Counsels the patient and family in meeting nursing and related needs. 9. Job Description ADVANCED PRACTICE PROVIDER – ORTHOPEDIC SERVICES (WALK-IN CLINIC - PRN) The Christ Hospital Health Network is in search of an advanced practice provider to join our musculoskeletal service line rendering professional clinical services to its patients as part of the general orthopedic, sports medicine and total joint replacement care team in the Greater Cincinnati area. This individual would be predominantly dedicated to the orthopedic walk-in clinic at our Montgomery outpatient location on an as needed basis (PRN). They will provide same-day access to non-emergent patients with acute orthopedic concerns. This is not a full-time position. Shift: PRN, Day shift Location: The Christ Hospital – Montgomery Outpatient Center (11140 Montgomery Road, Cincinnati, OH 45249). Job Overview: The Advanced Practice Provider (APP) may be a PA or NP that has been prepared with the appropriate graduate level education and authorized to practice and prescribe as such. The duties for this position include but are not limited to seeing acute orthopedic problems, follow ups, post operative patients, and surveillance of patients. This position will include outpatient office responsibilities. There will be no scheduling restrictions - the provider must be comfortable seeing any acute orthopedic related concern. The schedule will include PRN coverage as needed for the orthopedic walk-in clinic. There are no call or weekend requirements. Orthopedic experience is required. Job Description Patient Navigation refers to individualized assistance offered to patients, their families, and caregivers to help overcome health care system barriers and facilitate timely access to quality medical and psychosocial care from diagnosis through all phases of the disease experience. The primary goals of the Patient Navigator role are to: (1) improve patient outcomes; (2) support, educate, guide and empower the patient / family; (3) bridge gaps in communication and promote timely transition between service providers along the healthcare continuum; (4) provide seamless and efficient patient care; (5) remove barriers to care and minimize fragmentation of care for the patient / family; and (6) provide a consistent contact person and advocate for the patient / family, in order to improve the patient / family experience and satisfaction. Responsibilities Assessment Reviews baseline data related to patient: medical, psychosocial, spiritual, financial, and other patient needs. (This includes review of pathology and test results.) Assesses educational needs of patient/ family related to disease, treatment, side effect management, resources, etc. Conducts ongoing assessments of patient / family at specific identified time periods (e.g., diagnosis, beginning and end of treatment, recurrence, etc.) Assesses barriers to care for the patient / family, including health disparities, cognitive, psychosocial, financial, transportation, childcare, etc. Assesses community resources that are appropriate for the patient / family. Uses technologies (e.g., EMR, My Chart, etc.) to enhance communication among patients and health care providers. Serves as a professional resource to the team members in the Service Line regarding current trends in care, disease management, side effect management, and community resources. Participates regularly in onsite multidisciplinary patient conferences, consultations, and/or committee meetings (e.g., Tumor Board, etc.) Promotes evidenced based practice related to area of specialization and applies findings to policies and procedures. Plans, implements, and evaluates educational and/or supportive programs for patients / families, staff, and the community on appropriate topics. Participates in Community Activities, such as providing education at Health Fairs, church groups, etc. Remove Barriers: Assists patients to overcome barriers to care: financial, transportation, childcare Refers patients to supportive services, including financial counselors, dietitians, social workers, and community agencies Reaches out to underserved or rural populations. Create and utilize tools to measure the outcomes for the Patient Navigation program. Review outcome results to ensure that care is delivered to patients in accordance with national professional organization standards, guidelines, and benchmarks. Documentation – Documents recommendations made at multidisciplinary conferences, and distributes to appropriate multidisciplinary team members. Documents patient / family education in the patient’s electronic medical record. - Develop and utilize documentation tools to track encounters (e.g., follow up mailings, emails, phone calls, interactions, etc.) of Patient Navigator with patients / families. Leadership – Assists the Service Line Leadership Team with planning, implementing, and evaluating outcome studies, projects, and research related to the Patient Navigation Program. Collaborate with the Patient Navigation team in developing processes, documentation forms, patient educational materials, and patient / family education and support programs for the Patient Navigation Program. Demonstrates leadership skills such as the ability to motivate, educate, delegate, and mentor peers and other members of the healthcare team while effectively managing the care of patients and family members. This position does initial assessments, and regular re-assessments: provides ongoing nursing services, works as part of an interdisciplinary team, focuses on prevention, early detection, acute and chronic illness. In the event an RN from a physician practice works in an inpatient area, s/he will have the capability to write verbal orders, enter orders, and pend access notes, and will be required to attend physician THC training. Job Responsibilities Plans care for participants and family based on assessment, standards of care and optimal outcomes. Initiates and individualizes appropriate patient care guidelines, POC or clinical pathways. Maintains current plan of care by updating as needed. Develops both short- and long-term goals. Participates as member of Interdisciplinary Team by identifying caregiver and participant needs, recommending interventions and collaborating with other health care team members for mutually determined solutions. Performs technical skills according to hospital's and department's policies/procedures, appropriate competencies, and accepted standards of practice. Implements POC. Prioritizes workload according to participants’ needs and requests. Bases interventions on clinical data, desired outcomes, the nursing process, and standards of care. Documents interventions and outcomes. Utilizes critical thinking skills in daily care including accurately receiving, interpreting and executing physician orders. Evaluates the plan of care based on optimal patient outcomes. Documents the participant / family response to care including teaching. Collaborate with other members of the health care team to revise plan of care as needed. Supervises the care that was delegated to other health care team members. Demonstrates organizations responsibilities: Identifies areas for self-improvement, functions in relief charge / resource role as requested, completes assignments within scheduled timeframes, cooperates with instructors to facilitate effective learning experiences for students, maintains current knowledge in area of practice, demonstrates knowledge of organizational and unit changes. Conducts and documents comprehensive patient assessments with emphasis on promotion of health, prevention and early detection in order to plan and implement care directed toward optimal specific outcomes. Includes health counseling and teaching needs in assessments. Communicates assessment findings to other health care providers as appropriate.
