MAJOR RESPONSIBILITIES/ESSENTIAL FUNCTIONS Position Summary Reporting to the Manager, with direction from the Registered Nurse, this position is responsible for providing high quality, individualized basic care activities as identified by the RN, using the components of the nursing process. Performing duties in a manner that provides for the physical, psycho-social and spiritual needs of the patient including performing basic patient care activities. Incorporates cultural and ethnic factors into patient care activities within the limitation of the job, correctly differentiating and reporting normal and abnormal findings with direction from the RN and incorporates the functional/developmental age into these activities for the adolescent through geriatric patient population. Responding appropriately to concerns regarding patient safety based on the physical environment, equipment and the patient’s behavior. Demonstrating the ability to provide care in emergency situations and following established emergency procedures and practices, as appropriate. Communicating and following the organizational chain of command for notification of patient care/service issues, when appropriate. Maintaining knowledge of, and demonstrating the ability to identify patient care issues and assisting with patient care activities for the adolescent through geriatric patient population. Assisting with appropriate care through the healthcare continuum. Being aware of the reporting responsibilities to, and reporting suspected cases of abuse or neglect relative to the adolescent through geriatric patient population as appropriate. Reinforcing patient/family educational needs as identified by the RN and helping provide individualized basic teaching during the care giving process which considers the functional/developmental age and the needs of the adolescent through geriatric patient population. Observing and reporting the effectiveness of the patient/family teaching through observation of patient responses. Demonstrating knowledge of age specific care for the adolescent through geriatric population, including but not limited to identifying the need for additional safety measures, physiological normal values/readings; and assessing skin integrity, behavior, motor skills and/or activities that place patients at risk as well as communicates effectively in a clear, concise, understandable manner. CLINICAL • Wash hands before and after caring for each resident. • Place residents in correct and comfortable position. Head of bed elevated at 30 for all residents on enteral feeding. • Assist residents with dental and mouth care. Clean dentures after each meal. Keep dentures clean and soaked with Efferdent after dinner. • Record resident’s food and nourishment intake as directed. • Perform after meal care as required – cleaning resident’s hands, face, clothing, dentures, etc. • Assist residents to/from bathroom. Offer bedfast residents bedpans and urinals. Clean and store such equipment after each use (document Intake and Output as directed). • Check and document bowel movements and character of stools every shift. • Bathe residents as assigned and in accordance with established facility procedures (encourage showers and other self-help measures/activities). • Comb and brush resident’s hair daily. Shampoo hair on shower days. • Clip and trim fingernails. • Shave male residents daily or as needed. • Keep hair under arms and face on female residents clean-shaven as necessary or instructed. Trim hair in nose and ears as needed. • Observe and report the presence of reddened areas on bony prominences. • Keep drainage bag off the floor when resident is in bed or in wheelchair. • Apply Vaseline or A & D ointment to dry skin in accordance with established policies. • Move and turn bedfast residents at least every two hours and record such information on CNA notes. • Observe disoriented and comatose residents and notify Charge Nurse immediately. • Apply postural support properly. Check with Charge Nurse for type of restraint to be used. • Apply hand roll or splint to residents with contractures as outlined within the facility’s established policies and procedures. • Follow infection control procedures in accordance with established facility policy when isolation techniques are necessary. • Chart required information every shift. • Complete Intake and Output documentation on all residents with orders before the end of each shift. • Other related duties as may become necessary or as directed by the Charge Nurse, RN Supervisor, Director of Nurses, or Administrator. Works in a collaborative role with the management team, staff, and physicians in the clinical practices, as well as related departments throughout the organization. Prepare charts as needed, including filing into appropriate sections of the medical records, and assure that all pertinent items are available to the provider before patient care commences. Sets up exam room/procedure rooms appropriately based on patient visit needs; assist providers in examination/procedure rooms as needed. May fulfill back-office staff functions as needed including managing patient flow, charting, room prep, vitals, updating and maintaining medical records; and handling designated clinical care telephone interactions. As directed by the provider may perform designated diagnostic tests; draw, prepare designated lab work and/or specimens; administer immunization, injections, oral, or IV medications; convey lab results to patients; assist in follow-up scheduling needs; answer patient calls in a timely fashion and communication significant clinical findings to the provider.
