Duties & Responsibilities Purpose of Position : Under supervision, provides professional nursing care to patients/clients within an assigned unit. Diagnoses and treats responses to actual or potential health problems by means of nursing methods and techniques such as case-finding, health teaching, health counseling and provision of care supportive to and restorative of life and well-being. Job Description The CHHA Department is currently seeking a full time CHHA Registered Nurse to join their team! Eligible newly hired UFT nurses will receive a $6,000 hiring bonus, distributed in two phases: the first installment of $3,000 will be provided after successfully completing six months of employment, followed by a second $3,000 payment upon reaching one year of service. Description The Medical Assistant II provides clinical office support to physicians and surgeons and performs patient care and administrative duties necessary to deliver quality patient care. Mentors and assists less experienced staff members. Responsibilities Fulfills patient care responsibilities as assigned which may include: performing venipuncture and/or EKGs, checking schedules and organizing patient flow; accompanying patients to exam/procedure room, assisting patients as needed with walking, transferring and dressing, as well as collecting and processing specimens and point of care testing, preparing for exam, etc. Assists physicians with various procedures, takes vital signs, relays instructions to patients and or families; answers calls and provides pertinent information. Description The Case Manager (CM) will be responsible for all aspects of case management for an assigned group of inpatients to determine the appropriateness of the admission and continued stay, assist in the development of the plan of care; ensure that the plan is implemented in a timely basis and identify the expected length of stay (ELOS). The CM works collaboratively with physicians, social workers, clinical nurses, home care services, and other members of the interdisciplinary team as needed. The CM actively participates in specific clinical initiatives focused on reducing the length of stay (LOS), improved efficiency, quality and resource utilization. Assignment will be by units/clinical areas of practice and may require responsibility on other units/services Responsibilities Principle Duties and responsibilities include, but are not limited to: Reviews all new admissions to identify patients where utilization review, discharge planning, and/or case management will be needed using standardized criteria to achieve optimal patient outcomes and appropriate reimbursement for the organization. Performs continued stay reviews utilizing standardized criteria to justify continued inpatient stay. Collaborates with Physician and other clinicians to expedite diagnostic testing, treatment and consultations. Documents all clinical reviews in Canopy. Supports the mission, vision, philosophy and goals of the Medical Center. Case Management: The CM process will include: Assessment of the patient’s clinical, psychosocial, and functional status in collaboration with the interdisciplinary team. Identification and documentation of variances affecting the LOS and the discharge planning process. Conducts follow-up of any delays in treatment or reporting of results. Planning/developing specific goals with the physician, interdisciplinary team, and the patient and/or family. Implementation and coordination of specific activities, strategies, and interventions to move the patient through the continuum of care. Identifies reasons for readmissions and collaborates with interdisciplinary team on strategies to reduce readmission rate. Appropriately identifies and refers cases to the physician advisor to support timely progression of patients along the continuum of care and (appropriate) discharge planning. Utilization Review: Maintains a working knowledge of the UR requirements of each payor within the patient population; Provides the clinical information requested by the managed care companies as part of the concurrent review in a timely fashion. Description The Offsite Medical Assistant I Provides clinical office support to Physicians & Surgeons and performs patient care and administrative duties necessary to deliver quality patient care. Description The Milton and Carroll Petrie Department of Urology has long been a leader in clinical practice, education, and research. In this role, the successful candidate The Registered Professional Staff Nurse (RN) assumes responsibility for nursing care and outcomes for a group of patients for a designated time frame and provides care to these patients via the nursing process in collaboration with other members of the health team. Documents patient assessment reassessment progress toward goal achievement and all other required documentation accurately and timely. Promotes patient safety and quality including adherence to the National Patient Safety Goals Communicates and collaborates effectively with members of the health care team escalating situations appropriately. Establishes effective caring relationships with staff patients families and other members of the health care team. Administers medication and parenteral therapy in accordance with policy and procedure including blood administration. Establishes priorities for patient care delivery and uses critical thinking skills coordinates and completes patient care responsibilities independently or by appropriately delegating. Provides patient/family education and assists in discharge planning utilizing resources and considering patient/family readiness barriers and alternatives. Demonstrates knowledge and skills necessary to provide nursing care based on physical psychosocial educational safety and age-related criteria pertinent to the patients served in his/her assigned area. Additional Position Specific Responsibilities: The RN will be responsible to: ? Provide clinical assessment and care including wound care. ? Conduct health education sessions in individual and group settings. ? Assist with referrals and linkage to primary care, behavioral health, and specialty care. ? Link patients with substance abuse history to medication assisted treatment (MAT) or other treatment programs. ? Provide Overdose prevention tips and referrals to at risk clients. ? Encourage treatment readiness and adherence counseling; providing tools and tips for taking prescribed medications. ? Collaborate with DHS staff to ensure patients have continuity of care once they leave the shelter Minimum Qualifications: To qualify you must have a Must be a graduate of an Accredited Nursing Program, Registered Nurse License-NYS, BLS Certification through the American Heart Association.. Description The Clinical Nurse is a Registered Professional Nurse who provides safe, competent quality care based on nursing theory and research to a designated group of patients and significant others. Responsibilities Patient Care 1. Delivers patient care by incorporating the tenets of the professional practice model of Relationship Centered Care and through the application of the nursing process based upon theory, research, evidence-based practice, and approved organizational nursing standards. 2. Assesses and evaluates patient care needs and applies critical thinking skills in patient care management. 3. Integrates relevant assessment and intervention skills in the delivery of nursing care. 4. Collaborates with the interdisciplinary healthcare team in the development, implementation, and evaluation of the plan of care. 5. Prioritizes all aspects of patient care including teaching, rounding, coaching, and planning after hospital care and delegating to others as appropriate. 6. Educates patients and caregivers while anticipating needs and readiness to learn, about their plan of care, transition of care, promotion of health, and prevention of disease. 7. Communicates effectively and professionally with patients, family and all members of the Interdisciplinary Patient Care Team. 8. Manages assignments within the Care Delivery Model of Modified Primary Nursing and demonstrates allocation of material resources effectively. Patient Experience 1. Role models the tenets of Mount Sinai Relationship Centered Care (MSHRCC) through facilitation of the following actions including, but not limited to, Admission Welcome, Bedside shift Report, HELP (High Risk Medications, Equipment, Lines, Drains, Airway, Pain and Plan of Care), AM/PM Care, 5 Minute Sit Down, Purposeful Hourly Rounding, Medication Review, Discharge Wrap Up, and MD/RN Unit Collaboration. 2. Considers the patients values, preferences, expressed needs and knowledge in all aspects of care. 3. Establishes and maintains a therapeutic relationship with the patient and family. 4. Uses evidence-based practices to increase understanding of patient's perceptions of care. 5. Initiates service recovery for patients and family members who have concerns and escalates to Clinical Nurse Manager/Assistant Nurse Manager/Nursing Administrator or Patient Relations as needed. 6. Demonstrates caring, respect, compassion, empathy and active listening through dialogue, body language and actions. 7. Supports staff use of evidence-based practices to increase understanding of patient's perceptions of care. 8. Conducts Purposeful Hourly Rounding (PHR) on assigned patients addressing the 4 Ps. Quality and Safety 1. Practices safe and efficient patient-centered care, identifying and escalating barriers and opportunities for improvement. 2. Demonstrates clinical skills and knowledge crucial to quality and safety in the patient population served. 3. Implements process improvement strategies based on institutional, evidence-based ensure and procedures. 4. Engages in formal and informal peer and institutional review processes. 5. Collaborates with interdisciplinary teams to create and implement quality improvement projects, evidence-based practice activities and nursing research studies. 6. Contributes to quality and safety practices and required compliance measures. 7. Escalates potential safety hazards or gaps from best practice following institutional chain of command protocol. 9. Coordinates with the Clinical Coordinator/Clinical Nurse Manager/Assistant Nurse Manager/Nursing Administrator to ensure supplies are appropriately utilized and inventory is sufficient for patient care. 3. Escalates material or staffing resource deficits to charge nurse, Clinical Nurse Manager/Assistant Nurse Manager or Nursing Administrator. 4. Demonstrates patient focused and cost-effective approaches to patient care in terms of equipment, staff, supplies and all other resources. 5. Documents in an accurate and thorough manner in compliance with hospital, regulatory and legal requirements and standards of care. 6. Facilitates patient throughput on a shift basis.
