Description Job Title - Nurse Practitioner- Heart Hospital NPS - Mount Sinai Hospital - Full Time - Day A nurse practitioner (NP) as described in Title VIII, Article 139 section 6910 of the New York Education Law is a RN who has earned a separate certification as an NP through advanced clinical nursing education in a distinct specialty area of practice. Job Details St. Francis Hospital Critical Care Advanced Practice Provider (APP) is a valuable team member. Primary responsibility is the management of critically ill and complex (multi-organ) patients in an intensive care setting. This setting includes the management of patients in the Cardiothoracic ICU (CTICU), Surgical ICU (SICU), Medical ICU (MICU) and Intermediate Care Unit (IMCU). The APP is under the direct supervision of an in-hospital 24/7 Critical Care board certified Intensivist program. The APP performs physical exams, collects and documents data, conducts approved diagnostic and therapeutic procedures, orders and schedules laboratory studies and professional consultations, prescribes appropriate interventions and medications, and provides direct patient care services in a specific ICU. Description Position Title: Licensed Practical Nurse-Institute for Advanced Medicine-Mount Sinai West-Full Time-Days Position Summary: Participates in the application of the nursing process to an assigned group of patients. Demonstrates those behaviors which reflect the scientific knowledge, judgment, and technical skills of a Licensed Practical Nurse. Responsibilities A. Clinical/Technical/Service: Participates in the application of the nursing process to an assigned group of patients. Demonstrates those behaviors which reflect the scientific knowledge, judgment, and technical skills of a Licensed Practical Nurse Performs job duties under supervision of a Registered Professional Nurse. Demonstrates a competent level of approved skilled nursing care tasks and procedures using approved technical skills related to vital signs, pulse oximetry, venipuncture, starting standard peripheral IVs only (not midlines or long peripheral lines), urinary catheter insertion/catheter care; and simple dressings, remove/discontinue a peripheral line. Administer IV fluids, provided completion of annual competency IV therapy training. Monitors patients receiving blood and blood products and TPN under the direction of the RN, who administers the transfusion. Makes recommendations in the application of the nursing process. Participates in planning, and administers direct patient care, including activities of daily living (ADL). Performs point of care testing including blood glucose. Performs nursing procedures as defined in the nursing procedure manual. Administers medication as defined in nursing policy. ALL routes except cannot administer medications IV push/bolus (except for a Normal Saline or heparin flush) or via a central line. LPNs do not administer chemotherapy or immunotherapy. LPNs cannot administer first dose of an IV drug. The RN will administer the first dose of an IV drug and assess for side effects, after which the LPN can administer subsequent doses. LPNs working exclusively in chronic outpatient hemodialysis settings may, under the direct supervision of a Registered Professional Nurse (RN) and after completing an approved hemodialysis training and competency program, perform dialysis-related tasks as permitted by the New York State Education Department (NYSED) Office of the Professions and 10 NYCRR §700.4(b). See Section D for more details regarding role of LPNs in the Outpatient Hemodialysis setting. This exception does not permit independent practice or unsupervised access to central venous lines. Assists in patient assessment after the RN has completed the initial patient assessment, including pain management. Carries out medical and nursing orders. Cannot accept verbal orders or carry out RN standing orders. Makes, reports and records observations on assigned patients within the EMR. Observes and reports any signs and symptoms of change in a patient’s condition to the appropriate person. A Hemodialysis LPN may: • Administer bolus of medications by IV push in a chronic outpatient hemodialysis treatment center, excluding first dose administration which is administered by an RN. The RN will access and care for patient’s initial catheter use. Subsequently, and following the MSHS CVAD care policy, the LPN can: • Identify and set up supplies necessary for accessing a CVAD. • Initiate treatment with a CVAD. • Draw blood specimens from a CVAD. • Perform CVAD dressing change and exit site care. • Monitor and adjust flow rates of central venous catheter access device/line. • Intervene if there is a catheter/blood line disconnection or other complications associated with a CVAD. • Provide CVAD care at the end of dialysis treatment. Description The Medical Assistant II provides clinical office support to physicians and surgeons and performs patient care and administrative duties necessary to deliver quality patient care. Mentors and assists less experienced staff members. Responsibilities Fulfills patient care responsibilities as assigned which may include: performing venipuncture and/or EKGs, checking schedules and organizing patient flow; accompanying patients to exam/procedure room, assisting patients as needed with walking, transferring and dressing, as well as collecting and processing specimens and point of care testing, preparing for exam, etc. Assists physicians with various procedures, takes vital signs, relays instructions to patients and or families; answers calls and provides pertinent information. Fulfills clerical responsibilities as assigned which may include: sending and receiving patient medical records; obtaining lab/x-ray reports, hospital notes, referral information, etc. Completing forms and requisitions as needed, managing charts to ensure information is completed and filed appropriately. These responsibilities may be carried out using the electronic medical record (EMR) system or manually as necessary. Enters orders into the electronic medical record (EMR) system or prepares prescription refill requests on behalf of the physician for the review and approval by the ordering physician. Inventories, orders or re-stocks medical supplies as applicable. Provides set-up of examination room and instruments in accordance with proper sterilization techniques established by hospital and infection control policies. Maintains logging system as required for refrigeration of medications, point of care testing, sterilization process, crash cart or others as needed. Checks expiration dates on all medications and medical supplies disposing appropriately as per institution policy. Maintains a safe, secure, and healthy work environment by following standards and procedures and in compliance with legal regulations. Complies with established personal protective equipment requirements necessary for protection against exposure to blood and other potentially infections body fluids, chemical disinfectants, radiation and other hazardous substances. Assists in the maintenance of medical charts (filing, Op Reports, test results, home care forms). May schedule surgeries/procedures or work in conjunction with Surgical Coordinator verifying times with patients; preparing charts, pre-admissions and consent forms as necessary. May be required to perform proficient phlebotomy duties. May perform appointment scheduling and registration for patients as well as updating patient demographic and insurance information. May maintain, adjust and confirm patient office schedule. May check and verify & update insurance information utilizing internal or web sources. May perform and verify pre-certification for procedures as necessary Provides training and mentoring to Level I MA staff. Directs and assigns MA schedules and work flow to meet the needs of the practice and applicable to patient volumes. Oversee the successful delivery of clinical office support to meet the needs of each physician. Performs other duties and tasks as required. Qualifications High School Diploma/GED with completion of Training Certificate from an approved Medical Assistant certificate course. Certified Medical Assistant preferred. 2+ years of experience as a Medical Assistant For in-house candidates, promotion to Level II is contingent upon consistent E or S annual performance appraisals.
