How you make a difference The Nurse Supervisor is responsible for the direct supervision of the health care delivery system within their assigned nursing unit. They collect health status data, diagnose patients, set goals, plan and implement care, and evaluate patient progress. The Nurse Supervisor also provides patient education and ensures that ancillary personnel are qualified to perform the services they provide. Key Responsibilities Review and ensure compliance of nursing plans, medical records, and referrals with policies and procedures. Monitor and facilitate progressive patient care through team approach and patient education. How you make a difference The Nurse Practitioner provides on-site medical services by examining referred patients, conducting clinics, rounds, and responding to urgent and emergency situations. Additional Details Hours are: 7a-5:30p Candidates should be comfortable with substance withdrawal management, partnering with psych NP's and behavioral health staff, infectious disease management, MAT and standardized protocols (Title 15/NCCHC/ACA). In addition, candidates should feel comfortable doing suturing, incision and draining (abscesses are extremely common), basic wound care and debridement, splinting/minor fracture management, EKG interpretation, and emergency response (codes and overdoses) Qualifications & Requirements Education A graduate of an accredited school of nursing An advanced degree in an approved and accredited Nurse Practitioner program. Experience Prior medical/surgical and/or emergency/trauma experience and corrections experience is preferred. Must have the patience and tact to deal with the inmate population and ability to work effectively in the corrections environment. How you make a difference The Nurse Discharge Planner LVN develops and coordinates clinically informed reentry care plans for CalAIM-eligible individuals to address medical, behavioral health, and substance use needs prior to release. The role conducts basic nursing assessments, reviews health records, coordinates referrals, and facilitates warm handoffs to managed care plans and community providers. The position ensures timely service delivery by documenting services in the EHR, tracking release timelines, activating Justice-Involved aid codes, and collaborating with in-reach partners and Medi-Cal eligibility staff. Key Responsibilities Provide discharge planning services for medical, mental health, MAT, and related programs in accordance with contract requirements and policies. Identify release-planning needs, coordinate medications and medical needs upon release, and arrange referrals and appointments with community providers. Facilitate psychoeducational sessions, benefit applications, and resumption of services to support continuity of care. Maintain working relationships with facility staff, parole, mental health, and medical partners while adhering to security regulations. Document services in the health record, activate Justice-Involved aid codes, and ensure compliance with DHCS CalAIM requirements.
