Job Summary: Under physician’s orders and RN supervision, the Continuous Care (CC) Licensed Practical Nurse (LPN) is responsible for the observation of patient/family needs and providing supportive care and emotional support to the patient/family unit in times of crisis in order to maximize patient comfort and enhance quality of life.
