DESCRIPTION The On-Call Hospice RN is responsible for responding to all of the Hospice patient census and their families, as well as handling any referrals or community inquiries outside of office hour. The on-call nurse will provide urgent clinical and psychosocial interventions for patients and their families, skilled pain and symptom management and collaboration with providers within and outside of the hospice team to maximize positive patient outcomes. The work of the On-Call Hospice RN provides considerable consideration for independent assessment and judgement in crisis situations, utilizing and consulting with the Hospice Medical Director and administrator on call as indicated.

Atlantic Coast Rehabilitation and Healthcare Center, a 160-bed skilled nursing facility in Ocean County, New Jersey, is seeking an experienced MDS Director (RN) to lead our MDS program. If you have a strong background in MDS, care planning, and regulatory compliance, and enjoy collaborating with an interdisciplinary team, we'd love to hear from you. Key Responsibilities: Manage the scheduling, completion, and accuracy of all MDS assessments using Point Click Care (PCC) . Ensure compliance with all State and Federal MDS regulations . Submit MDS assessments electronically in a timely and accurate manner. Lead and participate in weekly utilization review meetings with the interdisciplinary team. Develop, review, and update individualized resident care plans. Educate and support staff on MDS processes and documentation requirements. Monitor Quality Measures (QM) and implement corrective actions to improve outcomes. Maintain accurate ICD-10 coding for residents. Participate in facility meetings and assist with other clinical responsibilities as needed. Compensation: $90000-$105000 per year and is based on your experience Qualifications : Current New Jersey Registered Nurse (RN) license in good standing (Required) . 2 years of recent MDS experience in a skilled nursing facility preferred. Experience using PointClickCare (PCC) requried Strong understanding of Medicare, Medicaid, MDS assessments, RAI process, and care planning. Excellent communication, organizational, and leadership skills.
