Offer Based on Years of Experience What You Need to Know Be the Best Palliative Care Nurse Practitioner You Can Be If you meet these qualifications, we would love to meet you: Graduated from an approved school of professional nursing One year experience as a Nurse Practitioner is preferred Licensed to practice as a Nurse Practitioner in the state of agency operation Active DEA license in the state where care is being provided. Possess and maintain valid CPR certification while employed in a clinical role, preferred Must be a licensed driver who can travel to all business locations Meet the regulations and requirements of the state(s) in which program provides services Responsibilities: As a Palliative Care Nurse Practitioner, you will: Develop and implement clinical plans of care for patients with chronic and terminal illnesses Perform patient assessment, history and physical; recommendations on aggressive symptom-management Monitor patient progress; update the Medical Director, Team Director and team members; consult regarding discharge plannning; and patient/family teaching Maintain communication with the supervisory physician (s) and attending physician to review patient records and care plans on a routine basis Initiate communication with attending physicians, other Palliative Care staff members and other agencies as needed to coordinate optimal care and use of resources for patient/family May provide support at time of death Prescribe medication as permitted by the state and/or the State Board of Nursing Participate in continum of care for patients that are hospice appropriate and ongoing education programs. Additional Information This position will specifically monitor post-operative and inpatient care of patients. Particular knowledge and care of these wounds is beneficial. The inpatient care will also include discharge planning and management under the guidance of the attending. This position will work closely with Residents and Fellows to care for the patients. This position will also attend to inpatient consults as requested. Assessment and escalation are critical components of this position. Credentials And Qualifications Bachelor's degree at minimum; master's degree preferred. Graduate of an accredited Physician Assistant or Advanced Registered Nurse program. Active FL medical license (or the ability to obtain one) and national certification by the appropriate accreditation body. Active BLS and ACLS. Bay Area Direct Client Care an immediate opening for an experienced home health aide/CNA in our medical group home. Ideal candidate will have experience working with multiple patients who are medically complex. Position is fulltime days 7am-7pm days needed rotate from week to week ex: ( Fri, Sat, Sun then Wed Thur, Mon, Tues) ( pay cycle runs Wed-Tue and paid out every Friday). Note training is paid at training rate of $15/hr. Once solo shift is initiated will start regular pay rate that is tiered between ($17-21/hr).
