Job Summary The Providence Center CCHBC Registered Nurse ACT II Clarity Team plays a vital role in assessing health needs, coordinating medical and psychiatric care, administering medications, and supporting overall wellness for clients. Working collaboratively with a multidisciplinary team, the Nurse ensures the delivery of high-quality, person-centered services that promote recovery, stability, and improved health outcomes in both office-based and community-based settings. Duties & Responsibilities Perform initial, annual, and ongoing nursing assessments, and update clients’ care plans. Identify expected outcomes of care, individualized to the client, with the goal of improving client’s overall health. Provide services to assigned caseload of clients, ensuring service intensity meets program standards, including required in-person contacts provided in both office- and community-based settings. Also provides services to other clients in need at Center programs. (See job description addendum) Triage clients and conduct comprehensive assessments, including health, psychiatric, substance use, housing, and safety needs. Identify, assess and respond to crisis situations, providing appropriate risk management and crisis stabilization. Develop, implement, and coordinate individualized care plans, including goal setting with clients. Manage care transitions, including hospitalization coordination and post-discharge follow-up. Communicate any changes or significant findings to care team, including prescriber. Provide integrated care aligned with medical protocols and question care plans when safety and/or wellness is at risk. Document assessments, lab results, medication updates, and required records in accordance with agency standards. Administer medications, ensure accurate EMR documentation, and complete medication reconciliations. Implement appropriately prescribed medication regimen while following the six rights of medication administration. Educate clients and families on diagnoses, care plans, rehabilitation, and medication effects and/or side effects. Uphold infection prevention, patient safety, and regulatory standards; report and escalate concerns per agency policies. Attend team meetings and case reviews to share updates and collaboratively address client challenges. Conduct home visits, school meetings, and community outreach to support clients in their natural environments. Assist clients with daily activities and life skills through modeling and providing education to promote independence. This includes basic medical procedures and hygiene needs as well as medication management and budgeting as appropriate. Maintain accurate, detailed, and timely records of all client interactions and services provided, including clinical documentation as required. Ensure compliance with agency policies, ethical standards, and regulatory requirements. Transport clients safely to appointments and activities as needed. Foster cooperative relationships with staff, clients, and community partners. Attend trainings and case presentations, and complete professional development activities necessary to maintain valid licensure. Participate in training and orientation of new staff and student nurses as needed. Serve on committees and participate in agency initiatives as assigned. Perform other duties as assigned to support client goals and program needs. Requirements Associate’s Degree Required; Bachelor's Degree Preferred (Diploma in Nursing acceptable). Field of Study: Nursing Experience working with psychiatric/mental health patients and substance use treatment preferred. Maintains a clean and safe working and/or living environment in the PACE center and/or participants’ homes. Assists with meal and snack preparation, serving, feeding as needed, and clean-up. Uses safe techniques in all interactions with participants at the PACE Center and in participants’ homes. Provides accurate and timely documentation in the EMR and other systems as required by SE policies and procedures and/or as assigned by supervisor. Participates in carrying out schedule of daily cleaning and disinfecting of the PACE center in accordance with CDC guidelines for increased precautions Actively participates in distribution of work for health aides to ensure care needs of participants and site tasks are completed timely and appropriately including but not limited to community-based care and center-based tasks, as assigned Actively participates in regular team meetings for health aides at respective site Participates in carrying out infection control precautions and increased cleaning and disinfecting of the PACE center as assigned and in accordance with current CDC guidelines Performs all duties in accordance with FH and Summit ElderCare policies and procedures. Qualifications Education: Completion of an approved Home Health Aide or Certified Nursing Assistant Training Program is highly desirable, but we will accept equivalent work experience in lieu of certificate. Certification: Certification as a Home Health Aide or Nursing Assistant is desired. Reliable Transportation Required. Experience: One year of experience working with a frail and/or older adult population preferably in a PACE program. If under one year, must complete Summit Eldercare training program for health aides prior to working with participants independently. Must complete a standardized set of Summit Eldercare competencies for Health Aides on an annual basis. Job Summary The Providence Center ACT Engagement and Diversion Team is looking for a motivated, team-oriented Registered Nurse, to help assess and facilitate the tasks and duties listed below for adults with co-occurring conditions, medical complexities, and who are also dealing with unstable living arrangements. Experience with the chronically homeless and addictions populations a plus, and within our new "CCBHC" (Certified Community Behavioral Health Clinic") capacity. Duties & Responsibilities Triage and assess assigned consumers on the team. Assess and evaluate client health care data collected through interviewing, observation, record review and team input. Develop an individualized consumer plan of care. Perform integrated care in accordance with appropriate medical protocol. Identify expected outcomes of care, individualized to the client, with the ultimate goal of improving client’s overall health. Implement the consumer plan of care. Evaluate the consumer's response to the plan of care and update the plan based on consumer's response. Provide support to consumers regarding daily living issues, conducting community and home visits as necessary. Instruct consumers and families concerning the disease process, plan of treatment and rehabilitation course, using available teaching materials when appropriate. Educate consumers on the various medications on the expected effects and side effects. Question any aspect of the consumer's treatment regimen which endangers the level of wellness. Administer medications as per prescriber orders (Injections and PO). Implement appropriately prescribed medical regimen while following the 6 rights of medication administration. Monitor lab results and complete related documentation Ensure that externally prescribed medications (from outside providers i.e. PCP) are accurately documented in EMR. Collaborate with the staff members in providing care to consumers. Perform recordkeeping in accordance with Agency requirements. Complete initial and annual assessment forms. Maintain cooperative relationships with consumers, staff and the public. Communicate with various local agencies, medical facilities, and other departments within TPC, including pharmacy. Attend trainings, case presentations and conferences as needed. Perform duties in accordance with departmental goals, objectives and standards. Serve on appropriate committees.
