Summary: Coordinates and facilitates the provision of high-quality, cost-effective hospice services through efficient triaging and communication. Supports clinical management to ensure appropriate utilization of resources as well as patient safety and satisfaction Schedule: Part-time, 24 hour days (3) 8-hour shifts - 7:30am - 4:00pm Rotating weekends Location: Baystate Medical Center, Springfield MA Job Responsibilities: 1) Demonstrates knowledge of applicable laws and regulations, developmental stages, pharmacology, health promotion and disease prevention and treatment, best practice interventions, Baystate Home Health and Hospice policies and procedures through daily interactions with staff, physicians, and patient/families 2) Receives phone calls and directs triage to ensure that patient's needs are prioritized and met, which may include an unscheduled visit or wellness calls 3) Receives and processes patient referrals, develops electronic medical records and collects necessary documentation to maintain regulatory compliance for a hospice admission; facilitates and complete communication as needed between patients/families, providers, and hospice team members; and provides educational and supportive information to patients and families related to hospice services and admission processes 4 ) Review reports and patient activity to prioritize admissions and routine visits. Work with management and team assistants to ensure efficient use of all staff with manager/clinical support and oversight. 5) Collaborates with finance and billing to ensure accurate payor source information is in electronic medical record 6) Assists in daily reports and communication of patients' needs to after-hours shifts. 7) Enter/edit/and/or process physician orders as appropriate in electronic medical records for updates in medication, equipment, scheduling, and plan of care needs. 8) Completes weekend scheduling in collaboration with clinical manager 9) Completes urgent visits as needed 10) Serves as a role model for effective communication across the organization. Supports clinical managers in visiting staff accountability 11) Coordinates smooth transitions of care for hospice patients between hospitals, homes, facilities, and other hospices. 12) Supports clinical staff with coordination of patient care needs including and not limited to DME, medications, supplies 13) Monitors utilization of resources and reports to the clinical manager 14) Assures accuracy of documentation in EMR including but not limited to visit times, orders, physicians, appropriate attachments, CIS accounts for the General Inpatient program 15 ) Timely communication with contracted vendors for patient services as needed, including enteral feeding, infusion services, etc. 16) Assumes a role in assisting with the development and implementation of policies, procedures, and standards of care for hospice patients. 17) Assumes accountability for own professional practice within ethical and legal parameters of nursing. 18) Works collaboratively with clinical staff and clinical manager to ensure timely communication within the team, referral sources, and throughout transitions of care 19) All other duties as assigned Minimally Required Experience: 3 years of hospice, home health or similar experience Preferred Experience: Electronic medical records, knowledge of hospice programs Education: GED or HiSET (Required) Certifications: Basic Life Support - American Heart AssociationAmerican Heart AssociationAmerican Heart Association, Driver License - OtherOtherOther, Licensed Practical Nurse - National Council of State Boards of NursingNational Council of State Boards of NursingNational Council of State Boards of Nursing Compensation Note: The compensation range(s) in the table below represent the base salaries for all positions at a given grade across the health system. Conduct point of care testing as needed (i.e., flu, strep, urinalysis, ECG, HCG); and provide patient information to provider . Responsible for stocking supplies and maintenance of exam rooms, front desk, and reception area; ensure providers have medical supplies needed. Responsible for completing order sheets to maintain supply inventory levels; regularly check expiration dates on supplies. Responsible for maintenance of medical records and scanning charts to patient files. Answer phone calls, collect and sort daily incoming and outgoing postal correspondence Ensure reception is well maintained Conduct courtesy call-backs for patient follow-up. Perform opening and closing duties, which includes cashing out, end-of-day communication, and securing the building. Follow operating instructions to perform and document daily controls and calibration of equipment; maintain Quality Assurance/Quality Control logs for equipment troubleshoot breakdowns, perform preventive maintenance, and submit repair tickets as needed. Provides a complete physical assessment and history of current and previous illness(es). 2. Provides professional nursing care by utilizing all elements of nursing process. 3. Assesses and evaluates patient’s status by: A. Writing and initiating plan of care B. Regularly re-evaluating patient and family/caregiver needs C. Participating in revising the plan of care as necessary 4. Initiates the plan of care and makes necessary revisions as patient status and needs change. 5. Uses health assessment data to determine nursing diagnosis. 6. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process. 7. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care. 8. Counsels the patient and family in meeting nursing and related needs. 9. The Team Lead for Utilization Review (UR) is responsible for the day-to-day activities and oversight of the UR Nurses, under the guidance and support of the Manager of Case Management and Director of the Center. Provides leadership and direction around scheduling, supervision and daily operations, while looking strategically at how workflow and process improvement can optimize and improve the role of Case Management and patient/family care through the institution. Lead will identify challenges and systems issues that need to be elevated to the Manager and Center Leadership. Team Lead-20%: Daily clinical support and supervision of UR nurses. Scheduling for UR coverage. UR Policies and Procedure review and maintenance. Onboarding and support of new staff. Individual and team supervision / support following challenging cases. Work with Manager to establish and track metrics, data and initiatives that improve process UR Team Communications. Assists Manager in completing performance evaluations. Provides formal and informal support of members through engagement activities, team supervision and staff meetings. Develop and maintain effective and efficient processes for determining the defensible hospitalization status based on regulatory and reimbursement requirements of various commercial and government payers. -20% Performs chart review of assigned patients to identify quality, timeliness, and appropriateness of patient care. Conducts hospitalization reviews for Medicaid beneficiaries, as well as other insurers and self-pay patients, based on appropriate guidelines. Uses these criteria to screen for appropriateness of level of care based on medical record documentation. -9% Gathers clinical information to conduct continued stay utilization review activities with payers pursuant to department policies and procedures and the Utilization Review Plan. -5% Escalates cases as appropriate for secondary review. -5% Performs concurrent and retrospective clinical reviews utilizing the appropriate guidelines as demonstrated by compliance with all applicable regulations, policies, and timelines. -5% Adheres to CMS guidelines for utilization reviews as evidenced by utilization of the relevant guidelines and appropriate referrals for secondary review. Identifies, develops, and implements strategies to reduce length of stay and resource consumption in conjunction with discharge planning staff. -9% Identifies and consistently documents information on any progression of care or patient flow barriers using the designated electronic tool used to track avoidable days/delays. -2.5% Engages hospital case management and care team colleagues in collaborative problem solving regarding appropriate utilization of resources. -5% Represents Utilization Management at various committees, as needed. -.5% Identifies and records episodes of preventable delays or avoidable days due to failure of progression-of-care processes. 2 Maintains appropriate documentation in the Utilization software system on each patient to include specific information of all resource utilization activities. -10% Provides consultation and education to physicians and other qualified practitioners regarding medical record documentation necessary to support the ordered level of care. -5% Conveys benefit data and options, programs and other forms of assistance that may be available to the patient, and negotiates for services as indicated. -.5% Communicates pertinent reimbursement information to healthcare team while observing patient right to confidentiality. -1.0% Verifies in-network verses out-of-network benefits and communicates data to the patient and healthcare team as indicated. -.5% Collaborates with other members of The Center team to coordinate the right care, in the right setting, at the right time for CT Children’s patients. Lead maintains current and accurate knowledge regarding commercial and government payers including regulatory requirements. Skills: Coordinates management of care for a specified patient population; follows patients throughout the continuum of care and ensures optimum utilization of resources, service delivery and compliance with hospital standards Provides ongoing support and expertise through comprehensive assessment, care planning, plan implementation and overall evaluation of individual patient needs. Skilled in the operation of the computer including proficiency in Microsoft Office Word, ability to use/update Excel spreadsheets and ability to navigate EPIC. Strong working knowledge of medical necessity tools such as InterQual® and Milliman Care Guidelines® and be proficient in medical record reviews. UR Lead will support process improvement activities and report key metrics to facility leadership as requested. The UR Lead will demonstrate an ability to effectively and efficiently manage a diverse workload in a fast-paced, rapidly changing regulatory environment. Lead will provide support to the hospital’s UM Committee as needed and collaborate with multiple leaders at various levels throughout the organization.
