Job Details APP- NP or PA – Cardiothoracic Intensive Care Unit (CTICU) FT – varied shifts (am and pm combined shifts) Position Overview: We are seeking a highly skilled and compassionate Advanced Practice Provider (APP) to join our Cardiothoracic Intensive Care Unit (CTICU) team in a renowned hospital in New York. This position offers an opportunity to work with critically ill patients undergoing cardiothoracic surgeries and to collaborate with a multidisciplinary team of intensivists, cardiologists, surgeons, nurses, and respiratory therapists. The ideal candidate will have experience in critical care and a passion for providing high-quality, patient-centered care in a fast-paced, dynamic environment. Registered Nurse-Intensive Mobile Team-Full Time Salary: $107,120 plus benefits Intensive Mobile Treatment (IMT) is an innovative, creative, trauma-informed mobile practice model designed to provide treatment, rehabilitation and support services to individuals whose needs have not been met by traditional services. IMT participants interact with homeless services, criminal justice, and behavioral health service systems. IMT teams are hired, trained and funded to maximize flexibility and continuity of care for persons who historically have been poorly served. IMT teams are multi-disciplinary and include psychiatry, nursing, social work, peer specialists and other staff members offering rehabilitation, treatment and recovery support services. The team provides expertise in the areas of housing, substance use and harm reduction, criminal justice, peer support and behavioral health recovery. Based on their area of expertise, team members collaborate to deliver comprehensive, integrated and flexible services that are responsive to a participant's choices, needs and goals. With a small staff to participant ratio, services are brought to the participant and offered in natural settings, 24 hours a day and seven days a week. Overview The Registered Nurse (RN) functions as a primary practitioner who assesses and monitors IMT participants' clinical health and response to treatment. The nursing approach of assessment, planning, intervention, implementation and evaluation is used to assists individuals in their achievement of mental health, rehabilitation and recovery goals. Develop, or consult and coordinate with team to develop, plans to address participant health needs. Monitor and modify treatment plans as indicated by participant responses and conditions. Collaborate with psychiatrist and other medical personnel to schedule appointments and coordinate care. Make referrals to community physicians and work jointly with hospital and institutional staff. Intervene and advocate on participant's behalf with, for example, collaterals, agencies, facilities and family members; assist with problem solving to help participant access needed supports. Provide consultation to IMT team regarding participant medical issues; serves as a resource to the team on medical concerns. Maintains accurate, detailed reports and records including required documentation, progress notes and health related data, in AWARDS. Evaluate and establish system for reporting on nursing intervention and expected outcomes. Attend and participate in supervision, meetings and training sessions as required. Provide 24-hour crisis intervention services on rotating basis. Job Qualifications Commitment to person-centered treatment strategies, upholding participants' rights, and self-determination in service provision. Currently licensed as a registered professional nurse by the New York State Education Department. Minimum two (2) years' experience working as an RN. Minimum two (2) years' experience working with persons with mental health condition in health or social services setting. Excellent written, verbal and interpersonal communication skills. Description The Medical Assistant I provides clinical office support to physicians and surgeons and performs patient care and administrative duties necessary to deliver quality patient care. This individual is also responsible for assisting with clinical, administrative and organizational tasks associated with patient care coordination. Responsibilities 1. Assists the provider during patient visit via obtaining vital signs (height, weight, blood pressure, pain scale etc) and documenting vital sign results and transcribing medication information into the electronic medical record (EMR). 2. Obtains prior authorizations for prescriptions and durable medical equipment 3. Drafts prescription requests in EMR and sends to provider for review, completion and processing 4. Performs Phlebotomy services inclusive of collecting and processing blood, urine and stool specimens and use of a centrifuge for spinning bloods 5. Performs POCT (w/the exception of glucose) and documents results in EMR for provider to review 6. Performs daily Quality Control for all POCT equipment 7. Orders clinical supplies when necessary 8. Maintains and stocks exam and consult rooms with clinical supplies and clerical forms used by Providers 9. Assists providers with procedures by preparing supplies and equipment and acting as a chaperone as needed. 10. Perform EKGs when ordered by provider 11. Performs pre-visit planning for Diabetic and CHF patients as per practice guidelines 12. Monitor and maintains refrigerator temperature logs and biomed expirations for medical equipment 13. Participates in daily huddles with MDs and other clinical team members to review daily care management patient lists and discuss patient's treatment plan & management 14. Assists Population Health Management team by verifying patient has completed their colorectal screening, breast screening, cervical cancer screening and other pertinent preventive screens. 18. Retrieves information regarding hospital admission and Emergency Room visits from outside facilities for evaluation by provider for transition of care coordination 19. Assists providers with follow-up related to patients on Coumadin therapy 20. Assists at the front desk as needed: answer telephones, register patients, discharge patients, schedule appointments, copy charts, schedule ancillary tests, and follow up on overdue diagnostic test results. 21.
