Follow facility established guidelines for reconciliation of medications on admission, during transfer and at discharge. Demonstrate least restrictive interventions appropriately. Collaborate with interdisciplinary team in assessing, planning, implementing and evaluating treatment, patient safety and programs. Assess patient’s response to treatment and safety interventions and accurately document interventions and patient’s response to intervention. Assist patients and families to understand the diagnosis, prescribed treatment, and safe management of symptoms related to specific mental disorders through health teaching.
