Overview Department: Nursing Administration Schedule: Nights, 7:00pm - 7:30am, 12-hour shifts Facility: Resurrection Medical Center Location: Chicago, IL 60631 Responsibilities The House Supervisor is a Registered Nurse who assumes administrative responsibility for the management of all department service functions on his/her shift. Makes frequent rounds throughout facility to monitor the delivery of care, oversee the quality of services and direct patient care activities as needed. Directly supervises, provides support to and communicates with all staff house-wide. In the absence of the Hospital Administrative staff, the administrative scope and responsibility is extended to all services within the hospital. Position Summary The Staff Nurse I provides direct patient care and takes the initiative to improve patient outcomes by integrating Shared Leadership, Evidence Based Practice, Relationship Based Care, and standards of nursing excellence into clinical practice. Duties & Responsibilities Completes required orientation program, unit-specific competencies, and cross training across oncology disease group specialties. Prepares for provider clinics by reviewing assigned provider schedules and patient assignments, assists with patient rooming, handles patient concerns, responds to EPIC In Basket, responds to patient communications, and performs oncology care coordination. Supports patient treatment and care plans, coordinating with Hematology/Oncology Infusion Center staff, and performing patient nurse visits. Verifies and implements orders from physician, physician assistant, and/or advanced practice nurse by performing interventions as prescribed by the provider and documenting accurately in the electronic health record. Shares information and collaborates with the healthcare team to promote effective patient care. Recognizes and treats cancer-related issues. Recognizes symptoms, establishes priorities, and responds to changing patient needs. Teaches patients and families regarding treatment expectations. Advocates for patients, families, significant others, the community, and the nursing profession. Supports quality outcomes, patient safety, customer satisfaction, and a clean, safe, therapeutic environment. Assumes role of Charge Nurse when assigned, utilizing appropriate resources and tools. Collaborates with other nurses to identify, promote, and provide positive learning experiences for staff and students. Serves as preceptor and/or resource to other nursing, support staff and students. Takes responsibility for continued professional growth/development by identification of learning needs and initiates strategies to meet such needs. JOB SUMMARY Responsible to provide direct nursing care to the customer, and to supervise the day-to- day nursing activities performed by the nursing assistants. Such supervision must be in accordance with current Federal, State, and local standards, guidelines and regulations, facility policies. Plan, develop, organize, implement, evaluate, and direct the Ladies and Gentlemen of Alden for a respective unit. Assume all Nursing procedures and protocols are followed in accordance with established policies. Directly supervise and instruct the Ladies and Gentlemen of Alden on a number of topics to promote job knowledge. Ensure all the Ladies and Gentlemen of Alden follow established departmental policies and procedures, including appropriate dress code. Assist in developing a written plan of care for each customer that identifies the problems/needs of the customer. Attends and actively participates in customer care plan conference as directed. Report all concerns, complaints made by the customer or their family to the Administrator. Counsel/discipline nursing personnel as requested or as necessary. Make daily rounds to ensure nursing personnel are performing required duties, and to ensure appropriate procedures are being followed. Make physical rounds to all customers daily. Make rounds with physicians as necessary. Create and maintain an atmosphere of warmth, personal interest, and positive emphasis, as well as a calm environment throughout the department. Attend and participate in continuing education programs. Recommend to the Director of Nursing the equipment and supply needs of the department. Place orders for medications and treatments as necessary following established budgetary guidelines. Utilize PCC 24-hour summary concerning recommendations/concerns related to customer care needs. Fill out complete accident/incident reports. Encourage attending physicians to review treatment plans, record and sign their orders, progress notes, etc., in accordance with established policies. Chart nurses’ notes in an informative and descriptive manner that reflects the care provided to the customer, as well as the customer’s response to the care. Complete required assessments upon the customer’s admission, transfer, and/or discharge. Receive telephone orders from physicians and record on the Physicians’ Order Form and P.O.S. Record new/changed diet orders. Forward information to the Dietary Department. Perform routine charting duties as required and in accordance with our established Charting and Documentation Policies and Procedures. Responsible for completion of admission, discharge, and transfer processes. Prepare and administer medications and treatments if appropriate as ordered by the physician. Review medication record for completeness of information, accuracy in the transcription of the physician’s order, and adherence to stop order policies. Administer professional services such as: catheterization, tube feedings, suction, applying and changing dressings/bandages, packs, colostomy, and drainage bags, care of the dead/dying, etc., as required. Participate in the orientation of new customers/family members to the facility. Arrange for diagnostic and therapeutic services, as ordered by the physician. Notify the customer’s attending physician and family when there is a change in the customer’s conditions. Obtain sputum, urine and other lab tests as ordered. Take and record TPR’s, blood pressures, accuchecks, etc., as necessary. Contact the customer’s physician for: Nursing assessment of change of condition. Medication orders or charges. To inform of incidents. To report abnormal results of Lab and X-ray. Anything else pertinent to the customers’ care and well-being. Ensure the nurses’ notes and monthly summaries reflect the care plan and is being followed. Attend staff meetings, etc., as directed. Perform other related duties as assigned. The on-call registered nurse organizes and directs hospice care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse directs the immediate services necessary to meet the needs of individual and families within their homes and communities. This position would help cover call for nights and weekends. This position is a Full-Time position working 7 days on 7 days off. The working hours for the 7 days on are: Monday-Thursday 5:00pm-8:30am; Friday 5:00pm - Monday 8:30am. Responsibilities: Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs for after-hours calls. Provides a complete physical assessment and history of current and previous illness. Available to receive and respond to calls from Triage during assigned shifts making visits to patients with changing conditions, and completing week end admissions. Provides professional nursing care by utilizing all elements of nursing process. Assesses and evaluates patient’s status by: Writing and initiating plan of care Re-evaluating patient and family/caregiver needs Participating in revising the plan of care as necessary Initiates the plan of care and makes necessary revisions as patient status and needs change. Uses health assessment data to determine nursing diagnosis. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care. Counsels the patient and family in meeting nursing and related needs. Provides health care instructions to the patient as appropriate per assessment and plan. Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care. After hours on-call responsibilities included remaining available for all calls related to patient care and making all appropriate visits as necessary. Remaining with patient and or family through a crisis period until resolved and or relived. Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate. Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required. Communicates with community health related persons to coordinate the care plan. Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary group. Provides and maintains a safe environment for the patient. Assists the patient and family/caregiver and other team members in providing continuity of care. Works in cooperation with the family/caregiver and hospice interdisciplinary group to meet the emotional needs of the patient and family/caregiver. Attends interdisciplinary group meetings. Ensures that arrangements for equipment and other necessary items and services are available. Supervises ancillary personnel and delegates responsibilities when required. Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes. Fulfills the obligation of requested and/or accepted case assignments. Actively participates in quality assessment performance improvement teams and activities. Qualifications: Licensed as a Registered Nurse / RN in the state you work in. Proficiency with Electronic Medical Records (EMR) software. Excellent planning, organizing, and communication skills. Ability to solve problems autonomously, ensuring responsibilities are met while maintaining high standards.
