The Nurse Practitioner will work closely with Dr. Md Shajjad Hossain and the broader multidisciplinary team, with progressive autonomy as clinical knowledge and experience within endocrinology develop. About the Role The Nurse Practitioner will provide comprehensive care to patients with a variety of endocrine conditions, with a significant focus on diabetes management and longitudinal follow-up. The position offers the opportunity to build specialized endocrine expertise while working collaboratively with experienced physicians and APPs. The NP will initially work closely with the physician team on more complex cases while progressively developing greater independence with routine endocrine follow-up care. The practice averages approximately 25–30 patient visits per day. Collaborating Physician – Dr. Md Shajjad Hossain The Nurse Practitioner will work closely with Dr. Md Shajjad Hossain, who will serve as the collaborating physician for this position. Dr. Hossain is committed to developing an APP who is interested in building expertise in endocrinology. The ideal candidate will be comfortable asking questions, receptive to feedback, and interested in progressively expanding their clinical knowledge and independence. Develop expertise in the evaluation and management of diabetes and other endocrine conditions. Review continuous glucose monitoring (CGM) data and make appropriate treatment recommendations. Adjust diabetes medications and insulin regimens based on clinical findings and patient needs. Support patients utilizing diabetes technology and provide ongoing education regarding disease management. Order, review and follow up on laboratory and diagnostic testing. Collaborate with physicians on complex endocrine cases and recognize when physician consultation is appropriate. Share responsibility for the clinical in-basket, including patient messages, test results, medication refills and follow-up needs. Provide clinic and hospital cross-coverage as needed. Educate patients regarding medications, diabetes technology, lifestyle modification and ongoing disease management. Maintain accurate and timely clinical documentation. Qualifications Graduate of an accredited Nurse Practitioner program. Current or eligible New York State Nurse Practitioner license. Appropriate national board certification. BLS/ACLS/PALS required. DEA certification required. Strong clinical judgment and communication skills. Ability to work collaboratively with physicians, APPs and other members of the clinical team.
