Demonstrate advanced practice skills with expertise in clinical decision making and independently incorporating concepts of standards in wound/ostomy/continence. Attend training and seminars to maintain clinical expertise. Serve as a resource for nurses, physicians and other health care professionals. Support Shared Governance Model. Quality Improvement: Assess, plan, implement and evaluate patient plans of care and on the best science, best service and best cost. Improve the outcomes of patients with wounds, ostomy and special skin care needs. Identify opportunities to improve core processes based on input from staff and customers. Document patient understanding/ outcomes of education provided. Provide feedback on effectiveness of the patient education. Monitor outcomes of patient/health education.
