Description The Evening Assistant Nurse Manager for Palliative Care is a Registered Professional Nurse with broad knowledge and skills who assumes the role of a clinical leader in the daily functioning of the unit and oversight of quality patient care delivery, in conjunction with, or in the absence of, the Nurse Manager. Responsibilities Patient Care 1. Leads nursing practice using the tenets of the Professional Practice Model of Relationship Centered Care and the Care Delivery Model of Modified Primary Nursing for the promotion of the patient experience. 2. The APP will provide advanced, evidence-based care to patients with acute and chronic heart failure across all stages of disease severity, including complex and advanced therapies. This role practices collaboratively with board-certified Heart Failure Cardiologists, cardiac surgeons, intensivists, nurses, and multidisciplinary specialists to deliver safe, effective, and compassionate inpatient care. This position is ideal for a nurse leader who excels in quality improvement, regulatory readiness, clinical oversight, and interdisciplinary collaboration.
