Job Summary Enliven is seeking an experienced RN (Registered Nurse) to service home infusion and Infusion Center patients in Pasadena and surrounding locations. The ideal candidate must be well-trained and able to give the best nursing care possible with little supervision. They will educate patients and their families on prevention and healthy habits. Provide hands-on care, management, and evaluation of the care plan and teaching of the patient under a physician based on ongoing assessments and as required by policy/regulation. Report patient care, conditions, and progress to the patient's physician, pharmacy, and Clinical Manager continuously. Implement patient care plan in conjunction with patient and family to assist them in achieving optimal resolution of needs/problems. Discharge patients after consultation with the physician and Clinical Manager, preparing and completing needed clinical documentation. Prepare appropriate medical documentation on all patients, including case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes. Prepare visit/shift reports, update/summarize patient records, and confer with other healthcare disciplines to provide optimal patient care. Job Summary and Qualifications As a Registered Nurse, you will be responsible for delivering high-quality, patient-centered care in line with the requirements of the department and the standards of practice for the relevant state and specialty. Collaborating with medical providers and the care team, you will provide personalized, comprehensive, and compassionate care, following established nursing models such as "Assess, Perform, Teach, and Manage." Assesses all “at risk” patients as identified by MEWS score increases Monitors surgery schedule and rounds on any “at risk” post op patients Facilitates debriefings of post codes Facilitate StaRN learning and serve as resource Enters RRT data and creates monthly reports using excel, reports data to appropriate committees Completes 24 hours post RRT/Code Blue follow up and documentation. Leads and facilitates RRT/Code Blue Committee Refers patient complaints to appropriate Unit Managers/Directors. Utilizes chain of command to ensure appropriate action is taken Facilitates a staff satisfaction feedback system for RRT What qualifications you will need: Graduate of an Accredited School of Nursing is required. BSN is preferred. Current Registered Nurse License in the State of Texas or Multi-State Compact License (Employees with RN Compact License are required to obtain Texas RN License within 90 days of hire date). Current BLS and ACLS Certification are required. NIHSS Stroke Scale Certification required. PALS preferred; required after six months completion in position.
