QUALIFICATIONS & ATTRIBUTES: Licensed Practical Nurse with current license in the state of employment. Minimum of one year of experience as a LPN in an acute care setting is required; two years is preferred. Home care experience is desired. MAJOR AREAS OF RESPONSIBILITY: Provides skilled nursing care in a patient’s home as ordered by the attending physician. Performs care by following order sets and goals made by RN. Observes and monitors patient conditions. Notifies RNCM of any changes in patient condition. Administers medication as prescribed by the physician. Counsels patient and family on the disease/injury processes and how to manage. Incorporates patient and family in development of plan of care. Helps decrease re-hospitalizations by teaching the patient on preventative measures and making good decisions. Listens to patient and family members. Completes all clinical documentation in accordance with agency protocol and Medicare/Federal guidelines. Documents all aspects of treatment, assessments, and patient education. Maintains active LPN license in state of employment. Communicates with scheduler any changes outside the normal. Participates in all on-call requirements and case conferences. Attends in-service trainings and mandatory agency meetings. Maintains a professional appearance as a representative of the company. Understands and practices agency policies and procedures. Other duties as assigned. ESSENTIAL FUNCTIONS: Drives to patient’s primary location per scheduled visit. Daily attendance at assigned visit locations. Documents all aspects of subsequent visits within 24 hours of visit to meet Medicare guidelines. Completes and submits all required documentation within specified company requirements. Knowledgeable with providing skilled care within the scope of practice for a Licensed Practical Nurse. Fulfills LPN visits in assigned geographic location per patient need within federal and state specifications. Educates resident on post discharge needs; Completes discharge paperwork; Informs nursing personnel of planned discharges for their shift; Arrange for discharge transportation Oversees the successful transfer of a patient Ensures all documentation is accurately completed for any admission, discharge and transfer Coordinates and evaluates the supportive interventions provided by nursing to other departments or services to achieve a holistic approach to resident care Coordinates and evaluates the efforts of other departments/services in providing education and support to family members in coping with and participating in the management of the residents' care needs Assists physicians, when necessary, during rounds and examinations Participates in the assessment, planning and implementation of nursing staff in providing resident care Participates in planning and implementing the coordination and involvement of nursing staff with other departments/services to teach family the management of the resident's care Monitors performance of nursing personnel to establish a high standard of care delivery and accurate documentation which reflects resident needs, progress, and condition Assists with sending requisitions to initiate physician orders Assists in planning, developing, directing, and teaching programs of in-service and continuing education for all nursing service and other personnel and directs other departments in the same, as necessary Takes positive action to ensure equal opportunity in the conduct of all business activities without regard to race, religion, color, age, sex, marital status, national origin, disability, or veteran status Maintains confidentiality of necessary information; Skills & Abilities Education: Successful completion of a state-approved pre-licensure nursing education program: Required Experience: One (1) year of experience in nursing and/or long-term care, preferred Computer Skills: Must be able to operate a computer, the Internet and knowledge of basic office equipment preferred Certifications & Licenses: Successful completion of a state-approved pre-licensure nursing education program; Active LPN license conferred by the State Board of Nursing; Must obtain and maintain mandatory, state and federal requirements and certifications for practice or occupation; Must be CPR certified Other Requirements: Good base knowledge of pharmacology, treatments, issues specifically concerning the elderly, dementia, various diseases, and side effects; Demonstrated people skills, with special attention to interfacing with patients" family members; Working knowledge of different cultures and religions; Must be familiar with/able to use the following equipment: WiFis, pagers, point click care, computer, sit to stand, hoyer, gait belt; Must be able to read, write, understand and speak the English language; Special care/dementia care employees will have additional training in caring for and treating dementia residents; Must be able to work with elderly residents, families and other health care staff members; Must be able to adhere to all terms and conditions set forth in the United Church Homes Employee Handbook.
