New Pay scale POSITION PURPOSE: Organizes and directs assigned unit duties to provide quality nursing care to residents. Forwards information to the Dietary Department. • Makes beds, bathes and feeds resident, when necessary. • Assures documentation is accurately completed for admission, discharge and transfer. • Supervises and evaluates implementation of the "Resident's Rights". • Participates in department or unit quality improvement activities. • Follows facility procedure requesting supplies and charges for items ordered through Central Supply. • Participates and attends periodic in-services and staff meetings. • Perform additional duties as assigned. EDUCATION/ REQUIREMENTS: Current State of LVN/LPN with a minimum of 2 years related experience; or equivalent combination of education and experience. Job Summary and Responsibilities As a Certified Medical Assistant (CMA) XRay Limited, you will be a vital pillar of our care team, significantly impacting patient well-being through direct support and seamless coordination. Under the direction of the health care provider, Medical Assistant – X-ray Limited is responsible for the delivery of patient care that promotes the safety and wellbeing of all patients. Accurately performs preliminary physical tests. The Director of Telehealth is the single accountable owner of this program: responsible for building and managing the clinical team, redesigning the platform and workflows that support it, launching and overseeing CCM/BHI operations, and ensuring the program meets its coverage, quality, financial, and retention goals. This is a hands-on operational leadership role. The Director will directly supervise a rotating team of Nurse Practitioners/Physician Assistants (FTE and PRN) delivering after-hours and weekend telehealth coverage, chronic care management, and behavioral health integration services across CareMed's partnered skilled nursing, assisted living, and long-term care facilities. POSITION PURPOSE: Work with physicians, implementing orders for medications, treatments and special tests. Supervise LPNs, LVNs and CNAs, providing them support in order that they may provide total nursing care for residents. ESSENTIAL FUNCTIONS OF POSITION • Utilize the nursing process to assess resident care plans, implement interventions, and evaluate outcomes related to nursing care. • Interview applicants, recommend hires; participate in counseling to extent permitted by law • Initiate and lead team conferences to develop individualized nursing care plans; assess and document resident's condition and nursing needs; assign team members who have the capabilities and qualifications to meet resident's needs • Implement physicians' orders; administer medications, start IVs, perform treatments, procedures and special tests and document treatment as required by company policy and local/state/federal rules and regulations • Ensure supplies are used economically and equipment is clean and maintained in a safe manner • Coordinate nursing care of residents scheduled for therapy or procedures by other departments • Report and record observations and reactions regarding residents • Assist or institute emergency measures for sudden adverse developments • Perform additional duties as assigned. EDUCATION/ REQUIREMENTS: Must be skilled in directing, motivating and training staff and able to work as a member of a team; able to communicate effectively with residents, family members and all levels of the organization and maintain confidentiality. POSITION PURPOSE: Organizes and directs assigned unit duties to provide quality nursing care to residents. • Responsible during the shift for the total nursing care of residents in the assigned unit. • Assumes responsibility for compliance with Federal, State, Local, and company regulations. • Explains procedures and treatments and care plan to resident. • Administers prescribed medications and treatments in accordance with approved nursing techniques. • Transcribes physician's orders. • Charts progress notes in an informative, factual manner that reflects the care administered as well as the resident's response to care. • Receives /gives report from/to nurse he /s he is relieving/coming on and records sufficient information and implements appropriate follow-up action as necessary • Follows established procedures for charting and reporting all reports of incident/accidents for res idents and employees. • Conducts frequent rounds to evaluate effectiveness of care delivery. • Prepares equipment and a ids Physician during treatment and examination of resident. • Maintains awareness of comfort and safety needs of resident. • Observes resident, records significant conditions and reactions, and notifies supervisor or Physician of resident ' s condition and reaction to drugs, treatments, and significant incidents . • Takes temperature, pulse, blood pressure, and other vital signs to detect deviations from normal and assess condition of resident. • Responds to emergency situations based upon nursing standards, policies, procedures, and protocol. • Assists in completion of the care plans (acute and long term) and is involved in the care conference as deemed necessary. • Participates, reports to DON/ Administrator and conducts (as appropriate) investigations related to any alleged abuse. • Ensures that documentation is reflective of the care plan and that the approaches on the care plan are being followed. • Assures that assigned CN A 's are aware of the resident care plan, and ensure that the nursing assistant refers to the care plan prior to administering care. • Monitors that CNA's documentation is completed daily . • Takes report form CNA's to facilitate accurate documentation in resident medical record. • Follows company policy and procedure in the event a restraint must be utilized. • Coordinates residents' rehabilitation program with therapists to ensure 24 hour rehabilitation needs are met. • Identifies residents at risk for nutritional problems and implements preventative measures. • Records new/changed diet orders. Forwards information to the Dietary Department. • Makes beds, bathes and feeds resident, when necessary. • Assures documentation is accurately completed for admission, discharge and transfer. • Supervises and evaluates implementation of the " Resident' s Rights". • Participates in department or unit quality improvement activities. • Follows facility procedure requesting supplies and charges for items ordered through Central Supply. • Participates and attends periodic in-services and staff meetings. • Perform additional duties as assigned.
