Overview Directs, coordinates, manages, and oversees the activities of the Medical Care at Home (MCAH) clinical staff to ensure the highest level of patient-centered care, including quality and financial outcomes. Collaborates with MCAH leadership and other VNS Health departments and stakeholders in the development, design, and implementation of care management program for the Practice.
