We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our Social Service Health Care Team. As a Registered Nurse Case Manager, you will be responsible for providing comprehensive patient care and coordinating services for the people we support. Your expertise in managed care, patient assessment, and medical terminology will be essential in ensuring the well-being of our patients. Key Responsibilities: Provide direct nursing care and clinical oversight for individuals with developmental disabilities in accordance with OPWDD guidelines. Administer prescribed medications and treatments, monitor for side effects, and maintain accurate medical records. Conduct health assessments and respond to medical emergencies as needed. Collaborate with interdisciplinary teams, including DSPs, medical providers, therapists, and family members. Develop and update individualized nursing care plans and contribute to person-centered service planning (ISP). Educate staff on health-related issues, proper medication administration, infection control, and other medical protocols. Administers medications when appropriate, and supervises AMAP certified staff in medication administration. Supervises all medication prescribed for individuals in accordance with agency policies. Provides supervision and training for Licensed Practical Nurses and/or less experienced. Provides immediate first aid with referral to outside emergency services or private physician. Description The Medical Assistant I provides clinical office support to physicians and surgeons and performs patient care and administrative duties necessary to deliver quality patient care. Responsibilities Fulfills patient care responsibilities as assigned which may include: performing venipuncture and/or EKGs, checking schedules and organizing patient flow; accompanying patients to exam/procedure room, assisting patients as needed with walking, transferring and dressing, as well as collecting and processing specimens and point of care testing, preparing for exam, etc. Assists physicians with various procedures, takes vital signs, relays instructions to patients and or families; answers calls and provides pertinent information. Description Job Title: Clinical Nurse-Med-Surge-SpineCenter-8C-Mount Sinai Hospital-FT/Days The Clinical Nurse is a Registered Professional Nurse who provides safe, competent quality care based on nursing theory and research to a designated group of patients and significant others. Responsibilities Patient Care Delivers patient care by incorporating the tenets of the professional practice model of Relationship Centered Care and through the application of the nursing process based upon theory, research, evidence-based practice, and approved organizational nursing standards. Assesses and evaluates patient care needs and applies critical thinking skills in patient care management. Integrates relevant assessment and intervention skills in the delivery of nursing care. Collaborates with the interdisciplinary healthcare team in the development, implementation, and evaluation of the plan of care. Prioritizes all aspects of patient care including teaching, rounding, coaching, and planning after hospital care and delegating to others as appropriate. Educates patients and caregivers while anticipating needs and readiness to learn, about their plan of care, transition of care, promotion of health, and prevention of disease. Communicates effectively and professionally with patients, family and all members of the Interdisciplinary Patient Care Team.
