The Vanderbilt Medical Center Staffing Collaborative supports nurse staffing needs as a system-wide resource. The Staffing Collaborative has created a new structure that allows alternative nursing employment at VUMC. Like a travel nurse, the Staffing Collaborative will allow Nursing staff to work 13-week assignments within a specific unit at VUMC. The Department of Nursing Strategic Programs and the Staffing Collaborative provide a new and exciting approach to nursing in a patient-focused, safe, and efficient manner. The assignment comes with premium pay and optional health benefits. The Staffing Collaborative's primary responsibility is to ensure coverage for the most significant scheduling needs across the organization. Assignments may necessitate working weekends and holidays. Minimum requirements will be consistent with the expectations for full-time staff within each specific unit. Job Types : Temporary during the duration of assignment, working full-time hours covering shifts most needed by the unit. This client requires full assistance with activities of daily living, including hands-on transfer assistance, and is considered a significant fall risk. We need a caregiver who understands that fall prevention requires constant awareness, close supervision, careful assistance, and proactive safety measures throughout the entire shift. The selected caregiver must be available to work all three scheduled days Monday, Wednesday, and Friday from 12:00 PM to 5:30 PM. Consistent attendance is essential because the client depends on a stable routine and familiar caregivers. The client also has a small dog in the home. Applicants must be comfortable entering and working in a home with a small household pet. At Helping Hearts, caregiving is more than a job, it is a calling. We are looking for someone who will treat the client with dignity, patience, compassion, and respect while remaining actively engaged and focused on the client’s safety. Primary Responsibilities Full Assistance with Activities of Daily Living The caregiver will provide respectful, hands-on assistance with the client’s daily personal-care needs as outlined in the client’s care plan. Responsibilities may include: Bathing or personal-hygiene assistance Dressing and undressing Grooming and oral-care support Toileting and incontinence care Mobility assistance Positioning and repositioning Safe transfer assistance Wheelchair and mobility-device assistance Meal preparation and hydration reminders General comfort and personal-care support Maintaining the client’s privacy and dignity during all care tasks Care must be provided patiently and professionally without rushing the client. The caregiver must follow the established care plan and use only approved transfer techniques and equipment for which the caregiver has received proper training. Safe Transfer Ability Is Required This client requires hands-on assistance with transfers. Applicants must have the physical ability and willingness to safely assist the client with transfers, including: Wheelchair to commode and commode to wheelchair Wheelchair to bed and bed to wheelchair Wheelchair to chair or other approved seating Sit-to-stand and stand-to-sit transfers Toileting-related mobility and clothing-management assistance Repositioning for safety, hygiene, and comfort Transfers using approved equipment when required by the care plan Previous experience using a Hoyer lift, sit-to-stand lift, or other mechanical transfer equipment is preferred but not required. Applicants without this experience must be willing and able to complete all required hands-on training and demonstrate safe use of the equipment before independently performing mechanical-lift transfers. During every transfer, the caregiver must: Explain the transfer process to the client Make sure the client is positioned correctly Properly position the wheelchair or commode Lock the wheelchair brakes before beginning Move footrests, cords, furniture, and other obstacles out of the way Clear and inspect the transfer area Use proper body mechanics Use approved transfer equipment when required Closely monitor the client’s balance and comfort Move slowly and avoid rushing or pulling the client Stop immediately if the transfer becomes unsafe Transfers must never be rushed or attempted in an unsafe manner. The caregiver must follow the client’s care plan and use any required gait belt, Hoyer lift, sit-to-stand lift, or other approved transfer device only after receiving proper training and authorization. If a transfer requires two caregivers or cannot be completed safely, the caregiver must not attempt it alone. The caregiver must keep the client safe and contact the Helping Hearts office immediately for assistance and further direction. Applicants must be able to perform the essential transfer responsibilities of this position safely and consistently, with or without reasonable accommodation. Proactive Fall Prevention and Safety Monitoring Because this client is at significant risk for falls, the caregiver must remain alert, attentive, engaged, and safety-focused throughout the entire shift. This is not a passive companion assignment or a position where the caregiver can sit nearby without actively observing and assisting the client. The caregiver will be expected to: Remain aware of the client’s location, mobility, and immediate needs Provide close supervision during standing, walking, toileting, and transfers Stay within a safe assisting distance whenever the client is moving Encourage the client to request assistance before attempting to stand or transfer Maintain a clear and hazard-free environment Check for spills, cords, loose rugs, clutter, poor lighting, and unsafe furniture placement Make sure mobility devices are properly positioned and within reach Make sure the client has appropriate footwear during mobility Keep commonly used items safely within the client’s reach Remain especially attentive when the client is tired, weak, distracted, or attempting to toilet Never leave the client unattended during a task when supervision is required Avoid personal-phone use or other distractions that interfere with active supervision Immediately report changes in mobility, balance, strength, or transfer ability Immediately report every fall, near fall, injury, or safety concern Complete all required incident documentation accurately and promptly The caregiver must never attempt an unsafe lift or transfer. Active Engagement and Companionship The client needs a caregiver who will be present, attentive, conversational, and genuinely engaged. Meaningful companionship is an important part of this assignment. The caregiver should: Initiate friendly and appropriate conversations Offer reassurance, encouragement, and emotional support Participate in activities the client enjoys Help maintain a calm, positive, and familiar daily routine Recognize when the client may be lonely, anxious, uncomfortable, or withdrawn Encourage safe participation in daily activities whenever appropriate Respect the client’s choices, preferences, personal space, and independence Remain patient if the client needs additional time, repetition, or reassurance Provide companionship without becoming distracted by a personal phone, television, or unrelated activities We are looking for someone who understands that quality caregiving includes both physical assistance and meaningful human connection. Job Responsibilities · Personal care: Assist clients with daily living activities such as bathing, dressing, grooming, and toileting. Meal preparation: Prepare and serve nutritious meals based on dietary restrictions or preferences. Mobility assistance: Help clients move safely within their homes or between locations, including transferring from beds, wheelchairs, or vehicles. Household tasks: Perform light housekeeping duties, such as laundry, cleaning, and organizing. Companionship: Provide emotional support and engage clients in meaningful activities, such as conversation, hobbies, or light exercise. Monitoring: Observe and report changes in clients’ physical or mental health to family members or supervisors promptly. Documentation: Maintain accurate records of care provided and daily activities Required Skills and Experience Maintain punctuality by arriving on time for all scheduled shifts and provide accurate documentation of daily care activities in client records. Strong communication and interpersonal skills to connect with clients and their families. Physical stamina to assist clients with mobility and other physical tasks. Ability to maintain confidentiality and adhere to HIPAA regulations. Demonstrate reliability by consistently arriving on time and completing assigned tasks with precision. Exhibit adaptability and problem-solving skills to address clients’ evolving needs. Uphold the values of the agency by providing care with empathy, professionalism, and integrity. · Ability to use a mobile device for clocking in/out and service documentations. Qualifications Must be 18 years of age or older Valid driver’s license and reliable transportation (for roles requiring travel). Able to pass a Criminal background check as well as status report on Abuse Registry and Sexual Offender Registry .
