Overview About the Role The Advanced Practice Nurse (APN) is a skilled health provider who through a written collaborative practice agreement with a physician is credentialed through the Medical Staff Office. As a member of the Medical Staff the APN demonstrates standards of professional practice that includes ethical decision making, job activities related to quality assessment/improvement, self-evaluation, leadership, and research. The APN is self-directed and purposeful in seeking necessary knowledge and skills to develop and maintain competency. The APN in the Surgery Practices utilizes critical judgment in the performance of comprehensive health assessments, differential diagnosis, and the prescribing of pharmacologic and non-pharmacologic treatments in the direct management of patients diagnosed with a variety of surgical conditions/diseases. The APN practice will promote wellness through conducting research, providing education and works autonomously as well as in interdisciplinary teams. Responsibilities What You’ll Do Collects comprehensive patient health data/history including past and present medical, surgical, family, and social history. Diagnostic tests and procedures relevant to the patient’s status are collected, interpreted, and documented at the Electronic Medical Record (EMR). Completes the medication reconciliation process by documenting the current list of medications (including over the counter and nutritional supplements) taken by patient at each visit. Performs and documents an appropriate comprehensive physical assessment of patients which will include a review of systems, evaluation of manifestations and toxicities related to medical conditions and anticoagulation management. In collaboration with the physician, critically analyzes the assessment data/diagnostic tests and determines a diagnosis. In addition, develops a comprehensive plan of care that includes interventions of treatments to attain outcomes. Evaluates patient progress in attaining expected treatment outcomes. Monitors for signs and symptoms related to disease progression/recurrence. Appropriately manages medical and anticoagulation management emergencies according to protocol/policy. Prescribes and/or orders medications. Obtains appropriate consents for surgery and procedures. Ensures completion of all needed pre-operative tests, forms, and documents. Documents assessments, prescribing, plan, modifications to plan, and evaluation of care in the EMR. Qualifications What You’ll Bring Required: Completion of an educational program which is designed and conducted to prepare graduates to practice as an NP/APN with a master’s degree in nursing and eligible for certification by a national certifying body. A current Registered/Professional Nurse license and registration in the State of New Jersey is required. Evidence of excellent interpersonal skills, effective communication skills, and creative problem solving are essential. A minimum of two (2) years of clinical nursing experience and current BLS certification are required. Responsibilities The Patient Throughput Coordinator will work with multiple entities to find and surmount the barriers to discharge of patient. This position acts as the designated liaison who facilitates and coordinates the transfer of patients into and within University Hospital. Works with referring agencies, hospitals, clinics, and the emergency department to ensure safe, timely transition of patients and clinically manages patient movement between units from the PACU, Emergency Department, outpatient clinics, procedural clinics and pre-admits for all service lines. Spravato (Esketamine) is an innovative treatment for adults with treatment-resistant depression. Demonstrates an understanding of, and embraces, the mission statement of the Christian Health. Schedule: Monday – Friday 37 hours per week. Partcipation in outpatient remote weekend coverage schedule (typically once every 6 to 8 weeks). Education: Must be a graduate of an accredited MSN program. Licensure: Current valid New Jersey APN and RN licensure required. Experience: Board certified in psychiatry preferred. Will consider previous experience as a APN board certified in addiction and/or trauma. Previous experience as an Advanced Practice Nurse in a medical and/or mental health setting strongly preferred. How you make a difference The Licensed Practical Nurse (LPN) plays a vital role in patient care by contributing to the nursing process of assessment, planning, implementation, and evaluation. Working under the supervision of RN(s), the LPN provides direction and guidance in patient education and activities that align with their education and demonstrated competencies. Key Responsibilities Under RN supervision, assesses patients' physical, psychological, and social dimensions in both the Health Care Unit and housing units, contributing to individualized treatment planning and consulting with RNs and staff as needed. Implements treatment programs directed by Health Care Practitioners, administering medications, including IV therapy (if certified), and performing diagnostic tests according to established protocols and procedures. Collaborates in controlled substance counts at the beginning of each shift, adhering to safety techniques, procedures, and standard precautions while responding to codes or health emergencies within standard guidelines. Assists in medical or minor surgical procedures, implements nutrition and therapeutic diet plans, and documents nursing encounters using SOAP charting, ensuring effective communication with nursing staff, physicians, and other relevant personnel. Attends mandatory staff meetings and training sessions, demonstrating a commitment to continuous professional development and adherence to policies and procedures in providing high-quality patient care. Additional Details Wellpath and Morris County Jail is hiring for a Licensed Practical Nurse (LPN) who is interested in working an 8-hour, full-time, evening shift (3p-11p), 40-hours/week (x5 shifts). The pay is dependent on years of experience and the range is $28.74 to $35.12 per hour and there is a shift differential that can be discussed during a phone screening. Qualifications & Requirements Education Graduate from an accredited school of nursing. Experience Prefer a minimum of one (1) year clinic experience Licenses/Certifications Have and maintain current licensure as a Licensed Practical/Licensed Vocational Nurse within the state of employment. Must be able to obtain and maintain CPR certification.
