Description The Nurse Manager is responsible for the management of nursing practice and operations on a designated unit/service. This includes, but is not limited to, the management of: clinical nursing practice, staff recruitment, retention, labor relations, supply chain and budget, staffing, employee performance evaluation, quality improvement, staff education, medical records, internal and external regulatory and survey requirements. The Nurse Manager collaborates with multiple professionals to support and coordinate the provision / management of patient care. He or she works closely with the Nursing Clinical Director, unit Dyad partner and staff in insuring that the Service/Unit maintains the highest standards of patient care and is responsive to the needs of the patient & family as well as the organization. Responsibilities PATIENT CARE 1. Leads nursing practice using the tenets of the Professional Practice Model of Relationship Centered Care and the Care Delivery Model of Modified Primary Nursing for the promotion of the patient experience. 2. Demonstrates proficiency in all aspects of Purposeful Hourly Rounding (PHR) and role models expected behaviors. 3. Ensures the integration of PHR into clinical activities to enhance Relationship Centered Care. 4. Validates staff members round hourly on all patients by direct observation and patient feedback. 5. Assesses staff competency in PHR and provides consistent meaningful feedback with more focused feedback as needed. 6. Conducts rounding of all patients on a daily basis (Monday-Friday.) 7. The assessment may take place in patients’ homes, hospitals or nursing facilities. The Hospice Field Admissions RN will follow the appropriate standards of practice established by the National Hospice Organization when admitting patients and insure the admission process meets current regulations. The RN will also engage in community outreach to physicians and other medical providers to share information regarding Hospice and Palliative care, as well as gather feedback to inform performance improvement initiatives. Must be able to cover Brooklyn or the Bronx. Access to a vehicle is required. Qualifications Graduate from an accredited School of Nursing, BSN preferred. A minimum of two years experience in either Medical-Surgical or, Community Health. Hospice or Oncology nursing preferred. Word and Excel. New York State RN licensure CPR-BLS and valid NYS Driver's license preferred. Ability to effectively communicate in a customer service environment. Ability to work flexible hours to meet the needs of the position. Ability to coordinate the resources and personnel involved in a comprehensive interdisciplinary team environment. Possess teaching, communication, and listening skills. Ability to work non-judgmentally with patient / family members of any culture, religion, socio-economic background or lifestyle.
