Must demonstrate knowledge of “five rights” of medication administration. Administers and records all medication given to residents’ based upon their specific chart. May take and record vital signs of residents as assigned or as may be required by certain medications. Certified Medication Technician CMT records the administration of medications and/or treatments appropriately, utilizing the computer system to enter and reference resident information as needed. Observes and reports any reactions and/or side effects of medications to a licensed nurse immediately. Administers PRN medications in accordance with state and federal regulations and consistent with facility policy. Ensure antiseptic techniques and good hand washing practices for infection control, as well as utilizing universal precautions are in place and practiced. Certified Medication Technician CMT reports and when appropriate, records any changes observed in condition or behavior of residents and unusual incidents. Submit incident/accident reports to supervisor or Administrator within twenty‐four (24) hours after their occurrence. Communicates with medical staff, nursing service and other department supervisors. Certified Medication Technician CMT supervises care staff in absence of nurses. Maintains, cleans and organizes the Medication Room. Maintains a liaison with other department supervisors to adequately plan for resident activities. Attends all required training, in-service and staff meetings May be subject to the handling of and exposure to hazardous chemicals. May be subject to exposure to infectious waste, diseases, conditions, etc, including TB and the AIDS and Hepatitis B viruses. Maintain resident confidentiality, staff confidentiality; treat staff and residents with kindness, dignity and respect. Comply with Resident’s Rights rules. Uses appropriate resources for patients with special needs such as language barriers, sensory problems, altered cognition, cultural and religious needs. Performs pain assessments and intervenes to optimally meet patient’s needs for comfort. Plans clinical care based on patient’s needs and uses available resources. Performs clinical care in accordance with policy and procedure. Performs peripheral venipuncture and peripheral and central vascular access care according to policy & procedure. Evaluates outcomes of nursing interventions and takes appropriate actions to move patient toward reaching goals of care. Identifies situations that warrant patient rescue and takes appropriate action, e.g. Rapid Response or Code Blue/Pink. Adheres to the 5 rights of medication administration and uses the Medication Administration Check (MAK) system per policy and procedure. Integrates principles of infection control & infection prevention into practice. Adheres to aseptic technique as appropriate. Uses resources and references for reviewing policies and procedures, medications, and clinical information. Prioritizes and organizes work effectively within assigned shift. Adheres to CCH Joint Commission Disease specific guidelines and core measures for the care of the patients with CVA Performs peritoneal dialysis according to policy and procedure. Performs med surg RN responsibilities for the patient requiring hemodialysis. Implements the nursing process for the adult and senior adult patient who presents with complex medical needs. Communication: Uses effective communication during hand-offs. (e.g. Shift report, transfer report, etc). Organizes information into a meaningful format, such as SBAR. Delegates appropriately to unlicensed personnel and team members. Communicates patient care information to the interdisciplinary care team. Provides patient and family education in a manner that is understood and gives the patient and family handouts, brochures and resources that can be taken home for future reference. Documentation: Documents patient care in an accurate, professional and timely manner using Soarian, MAK, and paper documentation forms according to policy and procedure. Documents patient care that is relevant to patient’s diagnosis, problems and reflects patient’s status.
