Nurse Practitioner for Residential Facility

Description Per Diem APRN APRN will report directly to the Medical Director. The APRN is responsible for overseeing the day-to-day medical flow. The APRN is responsible for completing admissions process with all new clients. The APRN identifies and documents chronic and acute medical needs of clients and communicates this to the nursing team. The APRN represents the facility in a positive manner, both personally and professionally. The APRN promotes a mutually supportive environment that emphasizes a collaborative approach and one which values contributions of all members, including staff and clients. Qualifications: APRN Licensure and Certification in the appropriate state, maintains current licensure per state requirements Malpractice insurance coverage Current CPR and Standard First Aid Certificate DEA license preferred Previous experience working in and a strong commitment to a team environment Previous work with addictions, mental illness, and homelessness or willingness to learn understanding of the systemic issues affecting those we serve preferred. Knowledge/experience of how to safely de-escalate potentially volatile situations. Current CPR and Standard First Aid Certificate Skills: Competent in using EMR and information systems in providing effective documentation of client care. Practice Registered Nurse Family Tree Warner- 36 hour benefit eligible

Hours - Tues- Friday 8-430pm with 1/2 day on Monday's 1-5pm Summary Reporting to the Clinical Leader or Clinical Manager and under the direction of the practice providers, the Clinical Practice Nurse 1 delivers high quality patient care including assessment, treatment, planning, and care evaluation. The Clinical Practice Nurse 1 uses knowledge of the principles of growth and development over the life span, assesses clinical data reflective of the patient's health status and interprets information needed to identify each patient’s requirements relative to his/her age specific needs. The Clinical Practice Nurse 1 engages in caring relationships with patients, families, and caregivers, works collaboratively with the health care team, and uses sound judgments while performing nursing interventions. Education Minimum requirements: Associate Degree or Diploma in Nursing from an accredited School of Nursing. Certification, Registration & Licensure Licensure required: New Hampshire Board of Nursing license Certification required: Current American Heart Association Basic Life Support for Healthcare Providers certification or Red Cross CPR/AED for Professional Rescuers and Health Care Providers Experience Demonstrated ability to effectively and positively interact with a multi-disciplinary staff, patients, and others. Responsibilities Establishes priorities in organizing daily activities and maintains efficient patient flow. Triages patient phone calls following established guidelines and prescribed format for identifying patient's health problems or concerns, and providing quality patient care. Administers prescribed medications. Provides education to patients in response to identified patient needs. Documents health information, interventions, and outcomes into the patient electronic medical record. Delegates patient care activities using the five rights: right task, person, direction, circumstance, and supervision (ensuring the task has been completed satisfactorily and documentation is complete) and providing feedback to delegatee and manager. Applies principles of purposeful, organized thinking based on the nursing process to make sound clinical judgment and decisions.
