Position Purpose This position provides face to face, virtual or telephonic care. Collaborates with their team members both clinical and non-clinical. Coordinates services provided for patients with chronic, or behavioral health/chemical dependency needs across the lifespan to improve the quality of care and satisfaction. Identifies social determinants of health and clinical symptomology needing intervention and works within the framework of the IDT to build a longitudinal plan of care and satisfy goals. Nature and Scope This position shall coordinate all components of Care Coordination services to provide for individual patients’ health care needs thorough the continuum of care. This includes Care Coordination which involves deliberately organizing patient care activities and sharing information among all the participants concerned with a patients care to achieve safer, and more effective care. This means patients’ needs and preferences are known ahead of time and communicated at the right time to the right people, and that this information is used to provide safe, appropriate, and effective care to the patient. The Care Coordinator will follow the Renown policies and procedures. The Care Coordinator will follow the Care Coordination Model of Care and Standard work as defined by CMSA. The scope includes potential for cross training within the department Care Coordination roles to cover for departmental vacations, illness and vacancies. Position Will Be Responsible For The Following Strong interpersonal communication skills both verbal and written. Remains productive and offers help and support to team members. Collaborate with the patient, family, providers and team members to develop a patient centered Plan of Care and support patient with self-management goals. Coordinates alternative community resources to include Home Health Care, REMSA, Durable Medical Equipment, Social Determinants and Community Partners to promote and assist the patient to have a safe environment of their choice and in alignment with the patient. Facilitate, problem solve with patients, families, providers and other health care professionals to effectively resolve patient care issues. Understands how to navigate Care Coordination process of Assessment, Planning, Goal Setting, Intervention, and Evaluation with the ability to utilize these components to provide for the individual health care needs and promote positive outcomes (quality). Helps with transitions of care and organizes medical information. Knowledge of applicable regulatory requirements and community resources Knowledge of continuous quality improvement process. Position Purpose This position provides optimal care utilizing knowledge and skills within the context of evidence based psychiatric nursing practice and the theory of human caring. Nature and Scope The psychiatric registered nurse on the Alert Team must be able to work interdependently within a team framework and demonstrate competency with mental health/chemical dependency assessment and referral. The incumbent must provide optimal care through assessment, planning and evaluation of adolescent, adult and geriatric patients and their families. The incumbent must be able to communicate effectively both verbally and in writing and must be able to initiate contact with internal and external resources on behalf of the client. Working knowledge of the 12-step recovery process, and the ability to empower clients to use their own internal resources as well as to access relevant external services is essential. This person must demonstrate the ability to work with patients and their families and various caregivers in different settings. This person must have thorough knowledge of community resources and be capable of making appropriate referrals. This position may require travel to other sites within the health system to conduct in-person patient assessments when virtual evaluation is not feasible due to the nature of the assessment or technical limitations. Bargaining Unit: Outpatient Clinics - EAP Rate of Pay: $80.87/hour + DOE Summary The Clinics Nurse Practitioner/Physician Assistant is responsible for the primary care of those individuals directed to the clinics via hospital based programs. Essential Duties and Responsibilities Demonstrates clinical competence to provide adult or family primary care with or without subspecialty. Provides primary care medical services for assigned patients, including preventive physical examinations, evaluation and treatment of injuries and illnesses, chronic disease management and referrals to appropriate specialties or facilities Orders, performs, interprets diagnostic tests as appropriate Prescribes medication, therapy and other specialized medical care per standard of care Performs appropriate medical procedures within scope of practice as defined by licensing and the health system's credentialing and privileging processes. Considers thorough range of treatment/preventive options and arrives at a recommended course of action that is age appropriate, culturally appropriate, realistic and attainable. Appropriately manages patient care and health status following initiation of treatment/preventive plan and monitors and adjusts as warranted. Responsibilities Northwest Specialty Hospital, an extension of Northern Nevada Medical Center , brings expanded services to a new building located at 6225 Sharlands Ave. in Northwest Reno. We are looking for a dynamic individual who provides direct and indirect patient care in the medical-surgical setting under the direct supervision of a registered nurse preceptor. Communicates with RN preceptor, as appropriate, about changes in patient’s clinical condition including results of diagnostic studies and symptomatology. Participates in the department's performance improvement and continuous quality improvement (CQI) activities. Must be willing to float to all units. Job Duties/Responsibilities: Completes Bedside shift report in the room at the beginning and end of each shift. Ensures patient is provided with the names and contact numbers for members of the care team on the communication board every shift. Consistently completes hourly rounds; 5 P's : pain, potty, position, periphery and alerting the RN to issues with the IV pump. C.N.A rounds with purpose including pro-actively evaluating the environment to ensure it is patient friendly and patient safe. Updates communication board when rounding complete. Performs patient care responsibilities considering the needs specific to the standard of care for the patient's age; pediatric, geriatric, and the general population. Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
