About the Role We are seeking a Solo Nurse Practitioner (FNP) or Physician Assistant (PA‑C) with acute care experience in Emergency Medicine, Urgent Care, or Internal Medicine to join our growing team. As a field‑based Advanced Practice Provider, you will independently deliver comprehensive care in patients’ homes—conducting assessments, diagnosing conditions, managing treatment plans, and coordinating follow‑up care.
