POSITION SUMMARY – Wound Care LPN 1199 UNION Position The primary purpose of the Wound Care Nurse us to provide primary skin care to residents under the medical direction and supervision of the residents attending physicians, the Nursing Supervisor, Director of Nursing or the Medical Director of the facility, with an emphasis on treatment and therapy of skin disorders. You will also assist in modifying the treatment regimen to meet the physical and psychological needs of the resident, in accordance with established medical practices and the requirements of this state and the policies and goals of this facility. Collaborates with regional staff as needed to work together toward mutual goals. CUSTOMER SERVICE Demonstrate positive customer service when performing the role of the Wound Care Nurse, with residents, family members, facility staff and medical staff. Display flexibility, team spirit, compassion, respect, honesty, politeness and accountability when dealing with residents, family members and facility staff. Demonstrates an awareness of and sensitivity for resident’s rights in all interfaces with residents and family members. Communicates effectively via open, straightforward communication, including use of listening skills and by accessing the appropriate chain of command. Seeks validation of knowledge base, quality decision-making and skill level by actively questioning when necessary. Participates in interdisciplinary communication activities; actively listens and actively contributes.

Role Description This is a full-time on-site role for a DON for a Hospice in Edgewater, NJ. The Hospice DON will be responsible for: An RN with clinical management experience preferred Responsible for RN case managers' oversight Triage experience Ability to communicate with families and fellow staff members Plan, prepare and execute staff in-service Responsible for CHHA education and disciplinary action Experience with chart auditing and QAPI standards Experience with CHAP survey standards CPR/ AED certified Hospice experience required Wellsky/Consolo EMR Record keeping of all staff medical records and licensing documents discharge planning, utilization management, nursing, medicine, and case management. Qualifications • Discharge Planning and Utilization Management skills • Nursing and Medicine knowledge • Experience in case management • Excellent communication and interpersonal skills • Ability to work in a team and independently • Bachelor's degree in Nursing or related field • Experience in hospice care is a plus • Current RN license in the state of New Jersey Benefits Insurance 401K
