No additional on call requirement SUMMARY: The Registered Nurse, On-Call will support a team of nurses and patients by responding to after- hours patient needs. They will have the opportunity to provide one on one patient care and work at the top of their license. They’ll be part of an interdisciplinary team that focuses on providing compassionate quality care and producing positive outcomes for our patient population. The RN On-Call will provide skilled nursing care in a patient's home as directed by the attending physician. If there are minimal to no after-hours needs during the shift, the RN On-Call will support chart review and patient phone calls from their home. They are responsible for all practices and duties within the scope of practice outlined by the state. KEY RESPONSIBILITIES Observe and monitor patient conditions and perform OASIS to develop an individualized care plan and adjust as needs change. Evaluates the treatments and medical condition of the patient according to the plan of care. Determines if the level of care being provided meets the patients’ needs. Administer medication as prescribed by the physician and help decrease re-hospitalizations by prioritizing visits for high-risk patients. Oversee and supervise total care of patient provided by nurse aides and LPNs. Manage multi-disciplinary care as applicable while promoting continuity of care with appropriate admissions , transfers, and discharges. Educate the patient and family on the disease process using teach back methods to ensure patient and family understanding. Complete all clinical documentation following agency protocol and Medicare/Federal guidelines. Completion of SOC/ROC/Recert and subsequent documentation within 24 hours. Collaborate with the interdisciplinary team and physicians to ensure optimal care is provided. Complete chart reviews and answer patient calls, to support patient care and regulatory compliance. Understand and follow agency policies, procedures, rules, and regulations and communicate changes in schedule/availability to schedulers or supervisors. Attend in-service training and mandatory agency meetings. Other duties as assigned. QUALIFICATIONS Minimum one-year experience as an RN in an acute care setting. Home care experience preferred.
