The LPN assists in assessing, planning, delivering, and documenting patient care, supports patient/family education, and contributes to the overall quality and safety of the healthcare environment. Care is provided in accordance with ACH standards, regulatory requirements, and within the scope of practice for LPNs. Responsibilities: Provide age-appropriate nursing care under the supervision of a Registered Nurse (RN) or designated provider, applying basic knowledge of biological, physical, behavioral, social, and nursing sciences to meet the needs of individuals and groups. Collects and monitors patient data, including packaging and obtaining specimens and labs, contributing to assessments and promptly communicating abnormal findings or changes in condition to the appropriate provider.
