Key Duties and Responsibilities - Conduct comprehensive assessments of patients' health status and medical histories. - Develop, implement, and evaluate individualized nursing care plans based on patient needs. - Administer medications and treatments as prescribed by physicians. - Monitor patients for changes in condition and respond appropriately to urgent situations. - Maintain accurate medical records using electronic health record (EHR) systems. - Provide education to patients and families regarding health management, including acute pain management strategies. - Assist in the Medication Room (Med Room), including preparation and administration of medications per established protocols. - Experience in Skilled Nursing Facilities (SNF) or Long-Term Care (LTC) settings is a plus.
