Position Overview We are seeking a compassionate and experienced Director of Nursing to lead our hospice clinical operations. This key leadership role is ideal for a Registered Nurse with strong hospice or palliative care experience and a heart for end-of-life care. The DPCS ensures exceptional care quality, regulatory compliance, and team coordination while supporting patients, families, and staff in a meaningful way. Key Responsibilities Clinical Leadership: Direct the delivery of hospice services in compliance with state, federal, and internal standards. Team Management: Recruit, train, supervise, and mentor clinical staff including RNs, LVNs, social workers, and home health aides. Care Coordination: Guide the interdisciplinary team in developing and executing personalized care plans. Regulatory Compliance: Ensure adherence to Medicare/Medicaid requirements and accreditation standards; manage audits and documentation. Quality Improvement: Drive initiatives to enhance patient outcomes and care quality. Budget Oversight: Manage clinical budgets and monitor resource utilization effectively. Family Support: Serve as a compassionate liaison for patients’ families, offering clear communication and emotional support. Cross-Department Collaboration: Partner with social services, spiritual care, and admin teams to provide holistic care. After-Hours Protocol: Develop and maintain readiness protocols for emergencies and after-hours needs. Qualifications Education: BSN required; MSN or advanced degree preferred. Licensure: Active Registered Nurse (RN) license in the state of California (required) . Experience: Minimum of 2 years in hospice or palliative care leadership. Overview Under the direction of a Registered or Licensed Vocational Nurse, provides basic nursing care and implements selected aspects of the patients nursing plan of care. This position requires providing care/service to an acutely ill through rehabilitating patient population in a manner that demonstrates an understanding of the functional/developmental age of the individual served. This position requires the full understanding and active participation in fulfilling the Mission of San Gabriel Valley Medical Center. It is expected that the employee will demonstrate behavior consistent with the Core Values. The employee shall support San Gabriel Valley Medical Center’s strategic plan and the goals and direction of the Performance Improvement Plan (PIP). Responsibilities Follows appropriate Code Blue notification procedures Effectively prioritizes clinical interventions and care Basic hygiene and nursing care such as: Feeding Bathing Oral care Positioning Ambulating Bed linen changes Patient safety measures Vital sign measurement. Intake and Output. Obtaining/transporting of specimens. Feed patients with swallowing difficulties following individual guidelines and orders from speech therapist Applies restraint devices, following all requirements for use of and documentation Non-sterile dressings/binders. Oral cavity suction. Obtains necessary training prior to initial equipment use. Assures equipment is in operating order prior to use. Uses the following equipment in accordance with manufacturer’s operating instructions. Including, but not limited to: Blood pressure measurement devices Thermometers Skin pressure relieving devices Pulse Oximetry Bedscales/ chairscales De-escalation of patients Adhere to “Patients’ Rights” Understand and follow LPS Law Complete Q15min monitoring of assigned patients Inventory patients belongings Notify RN/LVN of any changes in patients condition Other duties as assigned. Qualifications E DUCATION /T RAINING /E XPERIENCE State of California Nursing Assistant Certification At Least 6 months of experience in Psychiatric Nursing or Long Term Care (preferred) Excellent written and verbal communication skills Bilingual (English/Spanish/Vietnamese/Chinese) preferred Computer skills L ICENSES /C ERTIFICATIONS Current BLS (AHA) Current MAB (AB508) Training or within 3 months of hire Current CNA Certificate
