Under general direction of the CNO, the WCS RN coordinates treatment to provide comprehensive wound assessments and plan of care including goals, implementation of treatment, evaluation, and outcomes for patients throughout the facility. Provides patient, family and staff education regarding wound management, prevention of pressure ulcers, support surfaces, dressing selections and other therapies. Acts as the resource person for physicians and clinical staff for wound management. is able to perform general nursing duties in all departments with adequate supervision. Participates in quality improvement plan for the facility. Essential Responsibilities Conducts comprehensive assessment and continuous assessment of deep wounds, skin tears, ostomies, severe burns, pressure injuries or bedsores, continence and diabetic foot wounds to determine potential wound complications, immediate treatment needs, and long-term treatment plans; works with other members of the health care team to determine if additional treatments or changes in care are required. Collaborates with clinical providers and health care team to establish individualized, comprehensive plans of care for patients with complex wounds, ostomies, and/or continence conditions based on the patient's risks, diagnosis, and individual patient needs. Creates a plan of care for the interdisciplinary health care team, including the staff nurses, to follow. Completes a thorough wound care evaluation and documents assessments and treatment recommendations. Evaluates patient’s responses to care provided in conjunction with patient’s physician and modifies pressure ulcer /wound care /ostomy plan of treatment as needed. Provides staff, patient, and family education and counseling regarding wound care, risk factors associated with pressure injury, infection prevention, treatment measures, and other related issues; provides teaching, guidance, and direction to allied nursing personnel in rendering patient care. Refers other acute/chronic skin alterations to appropriate specialty services (i.e., Surgery, Vascular, Podiatry, Dermatology, Plastic). Develops and implements wound and skin care prevention and treatment programs, and provides corresponding education to patients, family members/caregivers, and health care settings and allied personnel; evaluates and monitors effectiveness of programs to ensure successful functioning and efficiency. Participates in product evaluation for ostomy and skin care needs for the health care setting to ensure cost effective, quality patient care. Participates in the development, evaluation, and revision of health care setting policies, procedures, and guidelines related to wound care, based on patient needs, evidence-based trends, and industry standards. Collaborates with appropriate staff to establish protocols that support best practices to achieve optimal clinical outcomes. Serves as a clinical expert to provide staff nurses with wound treatment protocols and assists in the education of staff nurses on how to assess wounds. Observes, records, and interprets patient response and wound status, reporting any changes to appropriate members of the health care team; records and maintains nursing care plans and progress notes on patients. Competencies Technical Ability to perform wound assessment, reassessment, and management for all patients. Demonstrates thorough knowledge of staging system of wounds. Demonstrates knowledge and skill to develop and implement policies and procedures to manage patients with draining wounds, fistulae and/or tubes, wound prevention, wound care, ostomy care, and incontinence. Ability to revise plan of care as indicated by the patient’s response to treatment and evaluate overall plan for effectiveness. The Advanced Practice Nurse (APN) provides medical assessment, diagnosis, and treatment of individuals experiencing substance-related withdrawal in a Withdrawal Management (Detox) setting. The APN functions under a collaborative agreement with a physician and is responsible for ensuring safe, evidence-based withdrawal management, medication management, and coordination of care in compliance with New Jersey Department of Health (DOH) and DMHAS regulations. Essential Functions Clinical Care & Withdrawal Management Perform comprehensive medical assessments upon admission and throughout withdrawal management. Diagnose and treat substance withdrawal syndromes, including but not limited to alcohol, opioids, benzodiazepines, and stimulants. Develop, implement, and adjust individualized withdrawal management plans, including medication protocols. Prescribe, administer, and monitor withdrawal-related medications (e.g., benzodiazepines, clonidine, buprenorphine, naltrexone, supportive medications) in accordance with scope of practice and collaborative agreement. Utilize standardized withdrawal tools (e.g., CIWA-Ar, COWS) and document clinical decision-making. Monitor for and respond to medical or psychiatric emergencies, including escalation to physician or transfer to higher level of care when indicated. Medication Management Conduct medication reconciliation on admission and discharge. Ensure safe prescribing practices, medication monitoring, and adherence to program protocols. Educate patients on medications, side effects, and treatment expectations. Collaboration & Care Coordination Collaborate with the Medical Director, nursing staff, counselors, and interdisciplinary team to ensure continuity of care. Participate in treatment team meetings, case reviews, and discharge planning. Coordinate referrals to ongoing SUD treatment, MAT programs, primary care, or higher levels of care. Documentation & Compliance Maintain timely, accurate, and complete clinical documentation in the electronic health record. Ensure documentation meets NJ DOH, DMHAS, Medicaid, and accreditation standards. Participate in audits, inspections, and quality-improvement initiatives. Adhere to all facility policies, infection-control standards, and patient-safety protocols. Education & Leadership Provide clinical guidance and education to nursing and clinical staff regarding withdrawal management. Support staff competency related to detox protocols, emergency procedures, and medication administration. Participate in policy development and protocol updates as needed. Collaborative Practice Agreement with a NJ-licensed physician. Master’s or Doctoral degree in Nursing (MSN or DNP) from an accredited program. Minimum Work Experience Minimum 2 years of clinical experience in substance use disorder treatment, detoxification, inpatient psychiatry, emergency medicine, or related setting (preferred).
