The LIFE programs offer an array of person-centered services from medical care to adult day supports with the primary goal of allowing individuals to remain in their own home. Under the supervision of the Director, Clinic and Home Care Services, the Registered Nurse RN - Home Care Coordinator provides a complete and thorough assessment of the home environment and the functional capabilities of each participant in the program. From this assessment and in collaboration with members of the Interdisciplinary Team, the Registered Nurse RN - Home Care Coordinator must make recommendations for home services. This position is responsible for establishing the services required and the time frames for the services. The Home Care Coordinator must be knowledgeable of the individual participant’s needs. Qualifications Registered Nurse RN - Home Care Coordinator requirements include: A registered nurse must be a graduate of a school of professional nursing. BSN Preferred. Minimum of one (1) years’ experience working with a frail or elderly population. Working knowledge of the interdisciplinary model of community care management. Experience working with cognitively impaired seniors is important. Must be able to relate well to seniors and their families to deal with sensitive issues and facilitate problem solving. Must be flexible and able to change easily. Working knowledge of computers, Microsoft Word and Excel required. Must have dependable transportation with a valid driver’s license in order to visit clients in their homes and other sites.
