Documents complaints on appropriate form. Investigates and develops solutions, gives feedback to resident/family. Keeps leadership informed. Implements plan of care for the resident based on assessments and goals as established by the interdisciplinary care team. Identifies and addresses psychosocial, cultural, ethnic and spiritual needs of residents and their families.
