Core Job Responsibilities • Support the PCP in the delivery of care by gathering and documenting relevant clinical data, as appropriate and per patient need. • Assists in specialty office procedures as directed. • Participate in the daily huddle to identify individual patient needs for upcoming appointments. • Clinical liaison between patient and provider for questions, concerns and testing results. • Communicate effectively with other members of the care team in coordinating the individualized plan of care. • Support PCP during in office procedures and examinations. • Provide outreach and education to patients in need of services as identified through quality improvement initiatives (Population Health Management). • Perform duties within scope of practice to the highest level of licensure. • Provide direct support & guidance to other members of the care team as appropriate. • Able to perform assessment data collection on all patients utilizing tools / forms in EMR. • Adequately assess and reassess pain. Educate the patient and family regarding pain management. • Provide age appropriate care to patients in the community setting. • Consistently document procedures completed in the EMR, to include medications administered and follow-up assessments • Deliver medication utilizing the five (5) rights and observes patient for desired untoward effects. • Maintain current knowledge of medications and their correct administration. • Establish priorities of patient care based on needs and office resources. • Demonstrates the ability to assist providers with procedures and perform services requiring technical and manual skill • Perform other duties as required. Education/Experience Requirements Required: • Graduate of an accredited school of nursing or nursing program. • Current New York State Registered Nursing registration. • BLS Certification. • Minimum 3 years of nursing experience • Basic computer and EMR knowledge. • Excellent communication and interpersonal skills. • Ability to assimilate information from many different sources. • Must be able to communicate in English both verbally and in writing.
