Functions as member of interdisciplinary healthcare team. Performs an initial discharge planning evaluation or Emergency Department assessment for patients. Provides supervision of Licensed Practice Nurse Navigator activities Cultivates and maintains effective interaction/communication with medical staff, nursing staff, social workers, and others to drive the care coordination process and facilitate continuity of patient care. Communicates with home health agencies, third-party payor, and other community resources as needed to coordinate needs. Facilitates clinical practice guidelines at the patient level. Performs outpatient and clinic care coordination and monitors patients' care amongst the hospital, clinic, and home. Facilitates care via telehealth to assist with patient needs and concerns. Provides health promotion activities and education for caregivers and staff at assigned facility, including workshops and trainings. Refers cases identified as risk management issues, peer review issues, or quality issues to appropriate personnel. Coordinates patient placement activities to optimize patient flow, patient outcomes, admission appropriateness, and cost efficiencies in support of organizational strategic initiatives while working with campus house supervisors, physicians, and nursing staff. Communicates and collaborates with leaders and staff at external organizations/agencies regarding placement needs for whole childcare. Maintains assigned databases. Collaborates in collection of data for identification and resolution of flow issues. Collects, analyzes, and interprets outcome data systematically to identify problem areas and assess need for improvement and/or support changes in system-wide processes in relations with the Behavioral Mental Health strategy. Prepares periodic reports as directed. Participates actively and contributes to organizational initiatives that impact patient flow. Supports continuous performance and quality improvement. Communicates openly with appropriate leadership team to ensure knowledge of key decision factors. Serves as expert clinical resource for patient placement by ensuring smooth functioning of the patient flow process to reduce wait times and avoid unnecessary patient movements. Maintains positive working relationship and demonstrates excellent customer services skills in all interactions between amongst the interdisciplinary team, physicians, internal customers, and external customers. Serves as a single point of contact for streamlining communication flow for escalated communication needs. Serves as a support liaison to the Behavioral Mental Health team. Triage & Vital Signs : Perform triage and take vital signs accurately. Documenting Patient History : Obtain and document detailed patient history in our Electronic Medical Record (EMR) system in a timely manner. Lab Specimens : Collect routine laboratory specimens, including blood, urine, and oral swabs. Medication & Injections : Administer medications and non-intravenous injections, including intramuscular, subcutaneous, and intradermal injections. Clinical Procedures : Start IVs, place catheters, and perform splinting when necessary. Clinical & Laboratory Procedures : Perform basic clinical, aseptic, and laboratory procedures to support patient care. Occupational Medicine : Assist with our Occupational Medicine services, including drug screening, breath alcohol testing, audiograms, and pulmonary function testing, while adhering to company protocols. Compliance : Enforce and maintain healthcare regulatory requirements, including HIPAA and OSHA compliance. Administrative Duties : Perform office procedures and general administrative tasks; proficiently operate office medical equipment. Quality Assurance : Oversee compliance with quality assurance programs, CLIA waived laboratory requirements, and patient result trackers. Travel Requirement: Support staffing and operational needs by traveling to other Piedmont Urgent Care locations as required.
