Job Overview Fairview has an immediate opening for a Medical Assistant to join our Vascular, Vein, and Wound team in Edina, MN This is a 1.0 FTE ( 80 hours per two week pay period), benefits-eligible opening. Clinic Hours of Operation are Monday–Thursday: 7:00AM–6:00PM and Friday: 7:00AM–5:00PM. NO WEEKENDS! While based on out Edina, there are times floating to the Maple Grove clinic will be required . This position is eligible for up to a $2,000 New Hire Sign On Bonus! As a member of the care team, the Medical Assistant supports quality patient- and family-centered care principles through performing a variety of high-level functions within a care team to support the needs of the healthcare team and patients/families. The Medical Assistant will gather information and documents to support the patient visit. The medical assistant is responsible for adhering to all policies, procedures and practice guidelines, promoting teamwork with all members of the health care team, maintaining complete, timely and accurate clinical documentation for each patient and continually looking for opportunities to improve processes and workflow. Responsibilities Delivers care as delegated by the physician/authorized practitioner or RN, to assigned patient population, or individual patient. Follows the patient’s plan of care. Prioritizes work based on patient acuity, available resources, patient/family preferences, provider schedule, in collaboration with the physician/authorized practitioner and other health care team members. Performs high and low risk technical procedures defined by the clinical setting includes medication administration, assistance with CPR, assistance with procedures and may include lab draws. Recognizes when different approaches may be needed based on age or cultural differences. Implements assigned aspects of the patient teaching plan. Documents care in the medical record. Effectively communicates compliance and concerns to delegating physician/authorized practitioner/RN. Takes action to assure continuity of care for all assigned patient. Adheres to all relevant policies, procedures, and practice guidelines. Responds appropriately to emergent situations Functions as care team assistant as needed Communicates effectively with other members of the health care team. Documents care in the medical record. What You’ll Do The RN – Clinic collaborates with providers to coordinate psychiatric services including medication refills for patients experiencing psychiatric/mental health disorders. Requires knowledge of nursing practices, which can be gained only through an accredited school of nursing. Oriented and educated in gender, cultural, and age appropriate care for the assigned clinic. Responsibilities and Accountabilities: Tracks and communicates pertinent medical and psychiatric information both verbally and through written documentation in EHR. Performs appropriate interventions as ordered by physician in an accurate and timely manner including needed testing such as lab draws, EKG, UDS, Genesight, and HcG testing. Documents results and provides safe handoffs to providers about the results. Provides care in a non-judgmental, non-discriminatory manner that is sensitive to the patient’s developmental age and to the patient’s and family’s diversity, preserving their autonomy, dignity, and rights. Maintains confidentiality in matters related to patients and families. Provides care management for patients over the phone utilizing psychosocial, physical, and behavioral assessments triage related to specific conditions and provides individualized directives based on these assessments. Provides assessment triaging for TMS patients. Maintains safe, secure, and healthy work environment by establishing and following standards and procedures. Administers medications and/or refills in an accurate manner consistent with policy and good nursing practice including verifying allergies and/or contraindications and monitoring for side effects. Administers long acting injections in a safe manner using safe nursing practices and consistent with clinic protocol. Completes medication refills. Educates patients and families about medications. Obtains insurance pre-authorizations for medications and tracks prior authorizations. Leads and delegates tasks to Licensed Practical Nurses and Certified Medical Assistants within scope of practice in clinics. Supports in scheduling patients for appointments when needed. Supports IOP group facilitation as needed. Assists in completing and coordinating referrals. Oriented and educated in gender, cultural differences and in age appropriate care for the assigned unit. Communicates effectively with interdisciplinary team members. Collaborates with team, seeks timely advice, and gives and always accepts feedback to ensure safe patient care. What Makes You a Great Fit Graduate of an accredited school of nursing with nursing degree. BSN preferred. Current licensure as an RN with the Minnesota State Board of Nursing is required. Evidence of 1 year of psychiatric/mental health nursing experience within the past two years preferred. Additionally, may have professional certifications, including but not limited to an RN ANCC Certification in Mental Health Nursing, Pediatric Nursing, and/or Holistic Health. Ability to intervene in pre-crisis or crisis situations by utilizing de-escalation techniques, including developing a plan of safety for patients as needed. Ability to provide remote coverage for clinics throughout the PrairieCare system. Must have critical thinking skills and ability to problem solve. Ability to communicate and make safe judgment calls in stressful situations. Ability to multi-task, use time efficiently, and prioritize appropriately. Ability to communicate in a professional manner in both verbal and written format and model positive teamwork attitude. Maintains confidentiality, remains open to others’ ideas and exhibits willingness to try new things. Must have basic computer skills using Microsoft Office products and EHR Ability to react quickly and calmly as part of a team to handle volatile and potentially emergent situations. The Assistant Director of Health Services Nurse is responsible for ensuring that the safety and health needs of the senior assisted living, memory care and enhanced care suite patients are met while providing staff leadership duties. Role is supported by a DHS RN, resident assitants/med passers, regional nurse, triage nurses. Reviews and updates service agreements at least once per year and more frequently when there is a change in service needs. Job Overview Riley Crossing senior living community located in Chanhassen, MN near Hwy 101 and County Road 212. Opened in 2019, it hosts 134 units of Independent, Assisted Living, Enhanced and Memory Care in total. This location also offers on-site intergenerational child care and interactive programming with our seniors. The Registered Nurse (RN) is responsible for ensuring that the safety and health needs of the residents are met. This Nursing schedule includes; Casual/On Call Day or Evening shifts Must pick up three shifts a month; two being weekend shifts Responsibilities Maintains current assessment and documentation of resident health status: Performs initial and on-going assessments per the applicable Minnesota Assisted Living regulations and Nurse Practice Act. Assures that all medication administration procedures are correctly followed. Takes appropriate action with staff regarding any problems or issues. Assures accuracy of medication and treatment records, including documentation of administration and appropriate signatures. Monitors work and performance of licensed and unlicensed staff: Meets with staff to coordinate care and address care issues. Plans and assigns/delegates work to staff within established guidelines. Interviews, trains, and orients new employees as directed. Assists and supports staff to complete assignments as needed. Assists Director of Health Services (DHS) in monitoring and evaluation of staff performance. Assists DHS in scheduling activities that support efficient and effective utilization of staff as needed. Schedules and completes Supervisory Visits. Complies with Federal, State and Local Laws. Support DHS to act as RN on call during scheduled times. Answers calls and gathers information related to assisted living community residents health and safety. Work collaboratively with indentified internal and external support providers. Actions taken may require phone directives and/or traveling to the assisted living community to resolve the situation. Required Qualifications Vocational/Technical Training or Associate Degree less than 1 year experience Proficient in Microsoft Windows 7 Registered Nurse RN Licensure/Registered with the MN Board of Nursing Basic Life Support (American Heart Assoc or Red Cross) Current Basic Life Support (BLS) for Health Care Provider from American Heart Association (AHA) or American Red Cross; accepted certifications as follows -AHA: BLS for Healthcare Providers (CPR & AED) Program or BLS Provider - BLS Instructor.
