Our state of the art brand new 21 bed Cardiovascular Intermediate Care Unit (3A) opened January 2025. We are looking for motivated nurses who are looking to professionally grow with us! Cardiovascular IMCU specializes in adult cardiovascular population including pre and post cardiac surgery patients, post structural heart procedures, post pacemaker/ICD placement. Post pulmonary and vascular procedures include but not limited to video-assisted thoracoscopic surgery (VATS), thoracotomy, fem-pop bypass; pericardial window, and endovascular aneurysm repair (EVAR). Nursing skills include competence in basic dysrhythmia, ACLS, and good critical thinking skills. About this Job: General Summary of Position The Medical Assistant performs clinical and administrative duties in an ambulatory care setting under the supervision of a physician/provider and/or a registered nurse. Position Title Registered Nurse (RN) or MSW Case Manager - Outpatient Clinic Job Description Purpose & Scope: The Case Manager is responsible for the collaborative process of assessment, planning, facilitation, and advocacy for options and services to meet an individual's health needs through communication and available resources to promote quality cost-effective outcomes. Additionally, the Case Manager ensures that identified and referred patients have a coordinated continuum of care in accordance with JCAHO and hospital guidelines and serves as liaison for the interdisciplinary team, the patient, and family to ensure a timely, smooth, and clinically appropriate care planning process. The Case Manager also maintains communications with the pay or at the time of admission and throughout the hospitalization to ensure necessary approvals for care and clarification of benefits. They facilitate retrospective reviews and approvals for care whenever necessary, and they implement and coordinate discharge plans utilizing available and appropriate community resources.
