Description The Offsite Registered Nurse is a Registered Professional Nurse who provides safe, competent, quality care based on nursing theory and research to a designated group of patients and significant others. Description RN/Case Manager MSH Case Management PT Days The Case Manager (CM) will be responsible for all aspects of case management for an assigned group of patients to determine the appropriateness of the Level of Care, assist in the development of the plan of care; ensure that the plan is implemented in a timely basis and identify the expected length of stay (ELOS). The CM works collaboratively with physicians, social workers, clinical nurses, home care services, and other members of the interdisciplinary team as needed. The CM actively participates in specific clinical initiatives focused on reducing the length of stay (LOS), improved efficiency, quality and resource utilization. Assignment will be by units/clinical areas of practice and may require responsibility on other units/services Responsibilities 1. Principle Duties and responsibilities include, but are not limited to: a. Reviews all new admissions to identify patients where utilization review, discharge planning, and/or case management will be needed using standardized criteria to achieve optimal patient outcomes and appropriate reimbursement for the organization. b. Performs continued stay reviews utilizing standardized criteria to justify continued inpatient stay. c. Collaborates with Physician and other clinicians to expedite diagnostic testing, treatment and consultations. d. Documents all clinical reviews in Case Management Documentation system. e. Supports the mission, vision, philosophy and goals of the Medical Center. 2. Case Management: The CM process will include: a. Assessment of the patients clinical, psychosocial, and functional status in collaboration with the interdisciplinary team. b. Identification and documentation of variances affecting the LOS and the discharge planning process. c. Conducts follow-up of any delays in treatment or reporting of results. d. Planning/developing specific goals with the physician, interdisciplinary team, and the patient and/or family. e. Implementation and coordination of specific activities, strategies, and interventions to move the patient through the continuum of care. f. Documentation of outcomes achieved and identified internal and external barriers. g. Identifies reasons for readmissions and collaborates with interdisciplinary team on strategies to reduce readmission rate. h. Appropriately identifies and refers cases to the physician advisor to support timely progression of patients along the continuum of care and (appropriate) discharge planning. i. Interacts with patient/family to discuss plan of care and coordination of services based on clinical needs and available resources. 3. Utilization Review: a. Maintains a working knowledge of the UR (Utilization Review) requirements of each payor within the patient population; b. Provides the clinical information requested by the managed care companies as part of the concurrent review in a timely fashion. c. Provides clinical information requested by the managed care companies in accordance with contractual agreements. d. Works collaboratively with physicians and managed care companies on concurrent denial appeals e. Communicates clinical information to the payor, as needed, coordinating direct communication between physician and payor Medical Director as required. f. In psychiatry, the CM will document UR notes in the medical record as per OMH guidelines. 4. Discharge Planning: a. Responsible for assessment, communication and monitoring of discharge planning process (The clinical nurse initiates the discharge planning process on admission). b. Obtains authorizations from managed care companies for post-discharge services. c. Responsibilities The primary purpose of the Staff Nurse position is to be accountable for the care activities, safety and goal achievements of assigned patients through assessing, diagnosing, planning and intervening in actual or potential health problems and evaluating patient’s response to the care. Responsibilities The primary purpose of the Nurse Anesthetist (Certified Registered) position is to administer anesthesia and anesthesia-related care in four general categories: (1) pre-anesthetic preparation and evaluation; (2) anesthesia induction, maintenance and emergence; (3) post-anesthesia care; and (4) perianesthetic and clinical support functions. Qualifications Graduate of an accredited Nurse Anesthesia Education Program with a Master’s Degree in a related field; or graduation from a program accredited by the Council on Accreditation of Nurse Anesthesia Education programs prior to January 1998 is required. Responsibilities The primary purpose of the Operating Room Technician II position is to be under the direct supervision of the professional nurse. The OR Technician II performs various duties to assist with surgical procedures and related operating room activities. The OR Technician II delivers care as reflected in the Association of Surgical Technologists’ Recommended Standards of Practice in a manner that is cost-effective without compromising quality of care. How you make a difference The Nurse Practitioner (NP) under the supervision of the Site Medical Director and other site Physicians, offers a comprehensive range of medical services to incarcerated patients in accordance with established policy and state guidelines. They ensure necessary documentation of services is provided to the Site Medical Director or designee to monitor the provision of clinical services. Additionally, they communicate scheduling changes to the Medical Director and H.S.A. and make arrangements for coverage of medical services if unavailable for an extended period of time. Key Responsibilities Provides medical care to inmates, including conducting exams, chronic care clinics, and infirmary rounds. Maintains accurate and legible progress note documentation in SOAP format and adheres to formulary guidelines. Responds to emergencies and makes logical decisions regarding non-formulary drugs or off-site referrals. Supports existing policies and procedures for admission, transfer, and specialty consultant referrals. Maintains confidentiality and provides input into facility committees as requested. Qualifications & Requirements Education A graduate of an accredited school of nursing An advanced degree in an approved and accredited Nurse Practitioner program. Experience Prior medical/surgical and/or emergency/trauma experience and corrections experience is preferred. Must have the patience and tact to deal with the inmate patient population and ability to work effectively in the corrections environment. Scheduling flexibility is also required to be able to rotate hours and shifts, if needed, and to be called during emergency situations to provide coverage. Licenses/Certifications Must have and maintain current licensure as a Nurse Practitioner within the state of employment. Must have and maintain current CPR certification.
