Responsibilities Key Responsibilities Plan, implement, coordinate, evaluate and promote the continuity of care consistent with established standards and practices. Assesses health status by interviewing patient regarding current status and medical history. Collects and records nursing assessment and interventions. Reviews and reports the results of the health status assessment and physical examination, pertinent laboratory data, radiographic data and psychosocial assessment of assigned patients to the provider. SUMMARY: The Nursing Supervisor/ Unit Manager performs professional supervisory nursing practice in the care of the physically and mentally ill. Work involves responsibility for supervising the provision of quality nursing care given at the facility. The position is responsible for maintaining the quality and adequacy of nursing practice; coordinating nursing programs; for recognizing and assisting in solving professional, administrative, and supervisory problems. Position exercises considerable judgment in applying professional knowledge in solving nursing problems within established policies and practices. ESSENTIAL DUTIES AND RESPONSIBILITIES: Determines the optimal allocation of present staff, per shift and assignment on each unit is present. Coordinates the care of patients on assigned shifts. Communicates shifts concerns by the unit to appropriate staff (via e-mails, voice mail, etc.). Review of 24 Hour Report and review patients as indicated for change of condition. Serves as the administrative authority for the smooth functioning of all units. Notifies the Director of Nursing, Assistant Director of Nursing, and Scheduler/Central Supply of urgent concerns during the shift. When the concern is non-urgent, notify the manager by phone message or e-mail. Receives employee, resident, and family grievances. Refers information to the appropriate manager. Assists nurses in completing risk assessments and investigations of injury. Notifies the Assistant Director of Nursing and Human Resources Director of identified learning needs. Receives employee call-offs. Records call-offs and updates schedule. Notifies managers/supervisors of call-offs in their units via voice mail. Replaces for absences and vacancies for upcoming shift(s). Assists staff with pick-up hours (call-in phone, e-mail, vacancy lists, 72-hour availability list). Administer all medications. Maintain a current and annual report of narcotics received and used. Daily review the documentation of dispensing controlled substances and narcotics. Reviews the records no less than weekly, or sooner if a discrepancy is noted, to ascertain that they are properly counted and documented from shift to shift utilizing EMR system. Order and give all new medications and chart same when done using the EMR system. Direct charting in his/her shift and make monthly detailed evaluation of all resident charting so that charts reflect progress and condition of residents in the EMR system. Be responsible for well-being and nursing care of all residents assigned to his/her unit while on duty. Contact families regarding significant decline in resident's status or transfer to hospital documenting contact in the EMR. Make rounds and observe individual residents who are experiencing episodes of acute illness, deterioration in health status, recent injury, recovering from surgery, etc., so as to be acquainted with the resident's status, both physically and emotionally and to ascertain that the staff is rendering proper care. At all times abide by policies of the facility and ascertain that employees under his/her supervision do the same. Supervise serving of prescribed diets and fluid intake (it is acceptable to document only deviations from normal) and report persistent unresolved problems to the DON. Ensure that all medications and treatments are charted by the person administering the medication or completing the treatment on his/her assigned shift (EMR system). Prepare incident/accident reports, events and observations using the EMR system. Ensure that a copy of the medical records or an abstract of them are forwarded with the transfer of residents to a hospital or other health care facility. Detect and correct situations that have a high probability of causing accidents or injuries to residents and/or staff. Assure that work/assignment areas are clean and free of hazardous conditions, i.e. spills, excess carts, etc. and that equipment, tools, supplies, etc. are properly stored while working, as well as before leaving such areas for breaks, meal times, and end of the work day. Assure that established infection control and universal precaution practices are maintained when performing nursing procedures. Follow established safety precautions when performing tasks and when using equipment and supplies. Report all equipment malfunctions and breakdowns to the ADON as soon as possible and keep the ADON informed of supply needs and equipment needing replacement. Maintain the confidentiality of resident information and honor his/her personal and property rights. Attend nursing department, Care Plan, and other meetings as directed by the DON or ADON. JOB SUMMARY: The purpose of this position is to assist the nurses in the providing of resident care primarily in the area of the daily living routine. QUALIFICATIONS: Must hold a current Certified Nurse's Assistant certificate in the State of Illinois. A high school diploma is preferred. Must have the ability and the willingness to work harmoniously with professional and non-professional personnel. Must be able to read, write, and speak English in an understandable manner. JOB REQUIREMENTS: Must be physically and mentally capable of performing routine, repetitive job duties. Show willingness to provide good nursing care. Be considerate, pleasant, and professional in the performance of his/her duties. Must have compassion, tolerance, and understanding for the residents of the facility. Knowledge of JCAHO, OBRA, IDPH, and HFS regulations as related to duties. MAIN DUTIES: Support the facility's philosophy of care and strive to achieve its goals and objectives. Be sensitive to residents' families and respond in an appropriate professional way as the situation requires. Carry out assignments for resident care including (but not limited to): a) bathing b) dressing c) grooming d) shaving e) feeding f) restorative nursing procedures g) retraining Keep residents' bed, dresser, bathroom, and general living area clean and tidy. Answer call lights promptly. Help check in all new residents and re-admissions, as well as assist in the transfer of residents to different rooms. Put resident's clothing away properly. Report any changes in resident's condition--e.g. eating habits, behavior, temperature, etc. to the charge nurse of the unit. Turn bedridden residents every two (2) hours or sooner as indicated. Wash, clean, and dry all incontinent residents. Assist the treatment nurse as assigned. Complete assignment report to charge nurse at the end of every shift. Be responsible for well-being and nursing care of all residents assigned to his/her unit while on duty. Take vital signs and monthly weights (with assistance) and record results in EMR. Document the following in the EMR for all assigned residents: a) vital signs b) weight c) meal monitoring d) bowl and bladder e) restorative records Detect and report situations that have a high probability of causing accidents or injuries to residents and/or staff. Assure that work/assignment areas are clean and free of hazardous conditions, i.e. spills, excess carts, etc. and that equipment, tools, supplies, etc. are properly stored while working, as well as before leaving such areas for breaks, meal times, and end of the work day. Assure that established infection control and universal precaution practices are maintained when performing nursing procedures. Follow established safety precautions when performing tasks and when using equipment and supplies. Assure that equipment is cleaned and prepared for the next shift. Assure that work schedules are followed. Maintain the confidentiality of resident information and honor his/her personal and property rights. Attend nursing department, Care Plan, and other meetings as directed by the DON or ADON. Attend and participate in in-service educational classes and on-the-job training programs as scheduled or as directed. Turn in Days Off and payroll special request forms in a timely manner. Follow established fire, disaster, safety, infection control, and evacuation policies and procedures. Perform other related duties as directed by the DON, ADON, charge nurse, or the Administrator.
