Responsibilities Perform comprehensive patient assessments and collaborate with facility staff on preventive services and early changes in condition recognition. Manage acute and chronic conditions, including medication adjustments. Order and interpret diagnostic tests. Develop, implement, and modify treatment plans as needed. Participate in quality assurance and improvement activities. Provide clinical education and training for patients, families, and facility staff. Adhere to state-specific NP scope of practice requirements. Demonstrates knowledge of etiologies, risk factors, pathophysiology, and presentation of acute and chronic conditions. Travel Requirement This role includes in-person patient visits within approximately a 25-mile radius of the clinician’s hiring home address, or as otherwise assigned based on business needs. Travel time during the workday is considered part of the role’s responsibilities and will follow all applicable wage and hour guidelines. Clinicians are expected to maintain a valid driver’s license, reliable transportation, and the ability to meet scheduling requirements for in-home visits. Geographic Assignment Service areas may be adjusted based on patient volume, care coordination needs, and operational demands. Travel expectations apply to all Nurse Practitioners hired under this updated job description. Existing Nurse Practitioners will maintain their originally assigned geographic expectations as part of a grandfathered arrangement, unless they voluntarily opt into the updated travel model.
