Overview Provides licensed practical nursing support to the Medical Care at Home (MCAH) clinical team during in-house and virtually supported preventive assessments. The LPN, Field Clinical Support is physically present with the patient and works under the direction of the Nurse Practitioner (NP), collecting and communicating objective clinical information, completing structured intake activities, supporting Care GAP closure, performing ordered procedures and testing within LPN scope and demonstrated competency, and reinforcing patient education. This full-time role supports individuals with dual diagnoses of intellectual and developmental disabilities (IDD) and severe mental illness (SMI) needs by integrating skilled nursing care, crisis stabilization, transitional planning, and intensive care coordination.
