Duties and Responsibilities: Assist the Director of Nursing Services in directing the day-to-day functions of the nursing activities in accordance with current rules, regulations, and guidelines that govern the long-term care facility. Review nurses' notes to ensure that they are informative and descriptive of the nursing care being provided, that they reflect the resident's response to the care, and that such care is provided in accordance with the resident's wishes. Provide direct nursing care as necessary. Start IVs, obtain sputum, urine and other specimens for lab tests as ordered. Take TPRs, blood pressures, etc., as necessary. Admit, transfer and discharge residents as necessary. Report problem areas to the Director. Assist in developing and implementing corrective action. Keep the Director informed of the status of residents and other related matters through written/oral reports. Assist the Charge Nurse in monitoring seriously ill residents. Ensure that all RNs and LPNs on your shift comply with written procedures for the administration, storage, and control of medications and supplies. Monitor medication passes and treatment schedules to ensure that medications are being administered as ordered and that treatments are provided as scheduled. Review medication cards for completeness of information, accuracy in the transcription of physician orders, and adherence to stop order policies. Inspect the nursing service treatment areas daily to ensure that they are maintained in a clean and safe manner. Ensure that residents who are unable to call for help are checked frequently. Meet with residents, and/or family members, as necessary. Report problems to the Director. JOB DESCRIPTION Job Summary Provides support for care management/care coordination long-term services and supports (LTSS)-specific activities. Collaborates with multidisciplinary team coordinating integrated delivery of member care across the continuum for members with high-need potential. Strives to ensure member progress toward desired outcomes and contributes to overarching strategy to provide quality and cost-effective member care. Essential Job Duties • Completes comprehensive member assessments within regulated timelines, including in-person home visits as required. • Facilitates comprehensive waiver enrollment and disenrollment processes. • Develops and implements care plans, including a waiver service plan in collaboration with members, caregivers, physicians and/or other appropriate health care professionals and member support network to address the member needs and goals. • Performs ongoing monitoring of care plan to evaluate effectiveness, document interventions and goal achievement, and suggest changes accordingly. • Promotes integration of services for members including behavioral health care and long-term services and supports (LTSS) and home and community resources to enhance continuity of care. • Assesses for medical necessity and authorizes all appropriate waiver services. • Evaluates covered benefits and advises appropriately regarding funding sources. • Facilitates interdisciplinary care team (ICT) meetings for approval or denial of services and informal ICT collaboration. • Uses motivational interviewing and Molina clinical guideposts to educate, support and motivate change during member contacts. • Assesses for barriers to care and provides care coordination and assistance to members to address psycho/social, financial, and medical obstacles concerns. • Identifies critical incidents and develops prevention plans to assure member health and welfare. • May provide consultation, resources and recommendations to peers as needed. • Care manager RNs may be assigned complex member cases and medication regimens. • Care manager RNs may conduct medication reconciliation as needed. • 25-40% estimated local travel may be required (based upon state/contractual requirements). Job Title: Certified Nursing Assistant (CNA) Job Summary: We are seeking a compassionate and dedicated Certified Nursing Assistant (CNA) to join our team at Hathorne Hill Rehabilitation and Healthcare Center in Danvers, Massachusetts. Responsibilities: Provide routine care to residents, including assistance with activities of daily living, such as bathing, dressing, and grooming Assist with serving meals, feeding residents, and meal documentation Maintain resident rooms in a clean and orderly manner Assist with scheduling appointments and transporting residents to activities and appointments Monitor resident conditions and report any unusual conditions, accidents, or incidents to the nurse Participate in informal and formal resident care meetings as requested Follow all health, sanitary, and infection control policies and maintain established standards of practice Requirements: C.N.A. certification as evidenced by completion of Nurse's Aide Training Program approved in accordance with the State of Massachusetts Public Health Code and a passing score on the MA Nurse's Aide Certification Exam Clean ASI registry check Previous CNA experience desirable, but not essential Ability to read, write, and comprehend English; ability to follow oral and written instructions Ability to work hours as scheduled based on the requirements of the position assignment Must be able to cope with the mental, emotional, and physical stress of the position Must be able to relate to and work with the ill, disabled, elderly, emotionally upset, and at times hostile people within the facility Physical Requirements: Must be able to move intermittently throughout the day, proper body mechanics required - bending, stooping, turning, stretching, and reaching above the shoulders are involved Must be able to see and hear or use prosthetics/equipment that will enable these senses to function adequately Must be able to lift a minimum of 50 lbs.
