Description The Offsite Licensed Practical Nurse I work as a part of the healthcare team by assisting in planning and implementing care interventions while promoting high quality patient care in a professional, efficient and safe manner, ensuring the patient flow. About This Opportunity This position requires a DSP with strong judgment, reliability, and relevant direct-care experience. The individual requires high level of support and supervision. The assignment rate reflects the complexity and expectations of this specific case. Responsibilities May Include Provide close one-to-one supervision and safety monitoring. Providing support with ADLs and daily routines as required. Follow individualized behavioral, medical, health, and safety protocols within DSP scope and training. Support communication, routines, community participation, and independence. Use approved support strategies consistently and calmly. Recognize and promptly report changes in health, behavior, functioning, or safety. Respond appropriately to urgent concerns and follow emergency procedures. Maintain accurate service documentation and dependable communication with the care team. Qualifications Prior experience in DSP, CNA, HHA, behavioral health, direct care, complex care, or a similar high-support role PREFERRED.
