Assist the L.P.N. in establishing routines that facilitate a high level of nursing practice. Receives and give reports from and to outgoing and incoming shifts. Count or ensure the counting of controlled drugs with outgoing and incoming shifts. Coordinates/Prepares MDS schedules for quarterly, annual and significant change assessments. Complete MDS Booklet as per schedule (Nursing Sections). Reviews and signs off on all completed MDS and RAP Summary. Develops care plan that demonstrate knowledge of criteria to ascertain resident specific needs based on health care problem(s). Makes needed revisions and updates to the care plans. Ensures that care plan is carried out which demonstrates familiarity with criteria to establish priorities specific to anticipated outcomes. Monitors the provision of care for the residents in his/her assigned unit. Interprets philosophy and objectives of nursing service to residents, families and others. Identifies resident/family learning needs based on the discharge care plan as applicable and carries out appropriate teaching required. Demonstrates ability to coordinate and collaborate with resident/family and other members of the comprehensive health care team. Monitors the administration and accurate recording of medications used. Investigates and follow through on medication errors. Notifies physicians of pertinent observations and reactions, incident and abnormal laboratory and x-ray results as required and documents same promptly. Transcribes physician orders timely and ensure orders are implemented as appropriate. Reports all resident, visitor and work-related accidents/injuries and demonstrates appropriate follow through by investigating, documenting and processing incident reports. Accepts guidance, supervision, and criticism if needed. Practices in compliance with the Nurse Practice Act and regulatory agencies regulations. Assures that a stock level of medications, medical supplies, equipment, etc. are maintained at adequate levels at all times. Assume the authority, responsibility and accountability of directing your nursing shifts. Other responsibilities as deemed necessary and appropriate or as may be directed by the Director of Nursing Services or Administrator. This position also performs clerical duties, acts as receptionist, maintains medical records and communicates with all departments. Job Duties & Essential Functions: Accurately collects clinical data on patients and demonstrates competency in performing clinical interventions and reports abnormalities to Medical Provider. Triage patient phone calls and troubleshoot medication management issues Documents nursing observations, interventions, and patient’s response to treatment in patient records. Demonstrated competency in performing venipuncture/administering shots for collection of blood specimens and facilitates the prompt and accurate processing and distribution of specimens collected. Ensures that patients receive timely and appropriate care within the delivery system and facilitates internal and external referrals in collaboration with the Registered Nurse (RN). Accurately performs and records vital signs, weight and height, point of service blood glucose testing, and promptly reports abnormalities to the medical provider or Registered Nurse (RN) on duty. Utilizes appropriate techniques when collecting routine urine specimens and specimens for C/S. Appropriately instructs patients on clean catch specimens. Utilizes appropriate technique in performing venipuncture to obtain blood specimens in accordance with agency procedure. Verbalizes knowledge of and adherence to OSHA standards for blood borne pathogens when handling blood and body fluids. Maintains standard precautions in the provision of patient care. Maintains and monitors stock inventory in the treatment center and replenishes supplies in exam rooms on a timely basis. Prepares and processes requisitions for routine supplies in accordance with agency procedures. Monitors the integrity of equipment and promptly reports malfunction, missing or damaged equipment to the Registered Nurse (RN) or Office Manager. Demonstrates competency in answering phones, patient registration, making appointments, computer data entry and retrieval of reports. Copies, prepares records, and stores and maintains appropriate records. Keeps records of all laboratory work sent, files laboratory reports on client record, and prepares and processes Lab/Diagnostic requisitions. Conforms to policies and regulations governing patient rights and confidentiality of information. Adheres to protocol for patient flow (1. Reception, 2. Registration, 3. Labs, 4. Escort to Provider). Perform all other related duties as assigned by management. Qualifications Required Education & Qualifications: Graduate of an accredited school of Practical Nursing. How you make a difference The Nurse Practitioner (NP) under the supervision of the Site Medical Director and other site Physicians, offers a comprehensive range of medical services to incarcerated patients in accordance with established policy and state guidelines. They ensure necessary documentation of services is provided to the Site Medical Director or designee to monitor the provision of clinical services. Additionally, they communicate scheduling changes to the Medical Director and H.S.A. and make arrangements for coverage of medical services if unavailable for an extended period of time. Key Responsibilities Provides medical care to inmates, including conducting exams, chronic care clinics, and infirmary rounds. Maintains accurate and legible progress note documentation in SOAP format and adheres to formulary guidelines. Responds to emergencies and makes logical decisions regarding non-formulary drugs or off-site referrals. Supports existing policies and procedures for admission, transfer, and specialty consultant referrals. Maintains confidentiality and provides input into facility committees as requested. Qualifications & Requirements Education A graduate of an accredited school of nursing An advanced degree in an approved and accredited Nurse Practitioner program. Experience Prior medical/surgical and/or emergency/trauma experience and corrections experience is preferred. Must have the patience and tact to deal with the inmate patient population and ability to work effectively in the corrections environment. Scheduling flexibility is also required to be able to rotate hours and shifts, if needed, and to be called during emergency situations to provide coverage. Licenses/Certifications Must have and maintain current licensure as a Nurse Practitioner within the state of employment. Must have and maintain current CPR certification.
